Orthostatic Hypotension and Albuminocytologic Dissociation as Primary Manifestations of the Paraneoplastic Syndrome

Abstract: Background: Orthostatic hypotension (OH) is the key manifestation of autonomic dysfunction with many causes. Systemic neurological causes such as paraneoplastic syndrome are usually ignored. Methods: We retrospectively analyzed clinical and examination data of 2 patients who were hospitalized, with onset symptom of OH and who were diagnosed as paraneoplastic syndrome. Results: The patients were characteristic of an initial symptom of OH, positive anti-Hu antibody and albuminocytologic dissociation in the cereb… Show more

“…50 Diabetes may be present in up to 40% of OH patients, as this is advancing age and chronic conditions, such as neurodegenerative diseases, hypertension, heart failure, diabetes, renal dysfunction, autoimmune diseases, and cancer. [19][20][21][22] The prevalence of age -dependent OH ranges from less than 5% in younger individuals (<50 years of age) to 20% and higher in those older than 70 years. 23,24 However, the majority of individuals who meet the OH criteria at screening are asymptomatic or unaware of the problem.…”

“…Although the negative Schellong test result did not confirm orthostatic hypotonia, the whole clinical picture supported the diagnosis. The described orthostatic disorders might also be due to prolonged stay in a recumbent position, insufficient fluid intake, the use of hypotensive drugs, hypochromic anemia, or as a paraneoplastic syndrome [28,29]. However, the onset of those symptoms after having had COVID-19 suggests the possible viral etiology of the syndrome.…”

“…We conducted a search for similar cases using "anti-Hu syndrome" as a keyword, but only a few cases or clearly related pathological findings were found. OH is rarely found as a comorbidity associated with subacute sensory neuronopathy, and to date, only two cases of OH with albuminocytologic dissociation have been reported in patients with paraneoplastic syndrome [8].…”
