Osteoarthritis: symptoms, signs and source of pain

Abstract: Osteoarthritis is the most common form of arthritis. The condition is characterised by loss or failure of the functional and/or biochemical integrity of the joint. The clinical symptoms include joint stiffness, pain and dysfunction, but the principal problem for the majority of patients is the pain. Although there are no pain receptors in the cartilage, the origin of the pain is thought to be due to stimulation of the A delta mechanoreceptors and the C polymodal nerve endings in the synovium and surrounding ti… Show more

“…Alternatively, it may be that pain is mediated differently in PTCL, as compared to FTCL. As cartilage does not have a nerve supply, in early osteoarthritis it may be that the pain is predominantly driven by inflammatory mediators as opposed to mechanoreceptors, and as such the response to treatment may be different as the disease progresses 21,22 . Finally, it is known that at post-mortem PTCL is a common finding in the asymptomatic knee and as such it must be acknowledged that despite a complete assessment in some cases of PTCL the pain may be referred other sites and the medial compartment may not be the cause of symptoms.…”

“…It is mainly considered to be the result of a bio-psychosocial network in which each of many biological, psychological and social factors plays a significant role (2, 3, [71][72][73]. Pain is considered a better predictor of disability than the degree of radiographic damage (74).…”

“…4 These changes lead to a reduction in the range of motion, and increase in joint stiffness and pain. 5 In addition to the effects on the joint structure and function, OA also has a negative effect on the musculoskeletal system. Patients with knee OA have decreased knee extensor strength compared to healthy subjects [6][7][8][9][10] and compared to the healthy contralateral limb.…”
