Otolaryngology Practice in Covid 19 Era: A Road-Map to Safe Endoscopies

Abstract: Covid-19 has changed the way medical services are being delivered all around the world. Otolaryngology as a speciality is anecdotally associated with high risk of infection. Endoscopies can be associated with aerosolization of particles due to cough or sneeze which may be induced. An overhaul of endoscopy and associated procedures is necessary keeping in mind the prevailing situations. This paper aims at a review of the ongoing research and development of a road map for safe endoscopies-both for patients and h… Show more

“…General precautions include using nasal pledgets or lignocaine viscous instead of sprays, standing at the back of the patient, flexible scopes via nasal route instead of rigid ones, video recording of procedure to avoid repetitions for review etc. 19 For OPD endoscopies, 45.1% wanted to use special drapes and screens and 41.7% would do the procedure after getting a COVID negative report. Quite a few (21.4%) were of the opinion of doing endoscopies for therapeutic purposes only and 28.5% will not be doing endoscopies rather use radiological investigations for diagnosis.…”

“…There has been much clinical discussion surrounding the use of FEES in the context of COVID given the fact that speech-language pathologists are at high risk for transmission of COVID due to the nature of the endoscopy assessment alone. Many experts in the field consider FEES or any endoscopic procedure to be an aerosol generating procedure (AGP) [ 1 , 3 , 4 ], while others do not [ 5 ]. While the literature does not provide a consensus on whether endoscopy is an AGP [ 6 ], the literature does agree that performing FEES puts providers at significant risk for transmission of COVID [ 5 , 7 , 8 ].…”
