Otoplasty Using a Modified Anterior Scoring Technique

Abstract: Otoplasty using a modified anterior scoring technique combined with postauricular fixation sutures provides long-term improved results in 91.2% of cases. Complications and sequelae are few, and rates are comparable to those of other long-term studies. A 2-mm lateralization of the auricle may be expected. Standardized measurements of the auricle are important because they supplement the overall aesthetic evaluation of the corrected ear and provide an objective baseline for long-term follow-up.

“…There are many procedures describing the incision of the anterior side of the antihelix to make it weaker and to fix the antihelix in the desired position with additional postauricular fixation, which certainly contributes to a positive long-term result [9] . Some earlier bending methods without incision, such as the Mustardé-type cartilage bending, led to poorer results in the long run, because the cartilage would spring back to its initial position after some time.…”

“… 7 All these techniques may be divided in two groups: with or without concha cartilage removal. 8 Resection of concha aims to eliminate the cartilage memory and reduces the tension between the sutures and consequently recidive. However, it can lead to lose of ear natural contour.…”

“…The overall complication rate in the pilot study was 20.5% (using 131 implants to treat 37 patients), which is comparable with the overall complication rate for conventional suture-based otoplasty techniques. 2 7 Further analysis of the outcomes led us to make changes to our technique for insertion of the implant and to the design of the Earfold introducer. To investigate the possibility that these changes might result in an improvement in the overall complication rate once the implant was put into routine use, we have performed an interim safety analysis describing the adverse events and complication rates in a series of 403 consecutive patients with prominent ears treated by 6 surgeons in the United Kingdom and 1 surgeon in Croatia.…”
