Our Definition of Propeller Flaps and Their Classification

Abstract: The term propeller flap was introduced for the first time by Hyakusoku to define an island flap, based on a subcutaneous pedicle hub, that was rotated 90 degrees to correct scar contractures due to burns. With the popularization of perforator flaps, the propeller movement was applied for the first time to a skin island vascularized only by an isolated perforator, and the terms propeller and perforator flap were used together. Thereafter, the surgical technique of propeller flaps evolved and new applications de… Show more

“…2 In the new comprehensive classification of propeller flaps, three more subtypes, axial pedicled propeller flaps, muscle propeller flaps, and chimeric propeller flaps, were added to the previous classification. 3 In a perforator-based propeller flap, the perforator is chosen near the defect. The flap is islanded based on the single perforator originating from a known underlying vessel.…”

“…19 Its rotation can range from 0°up to 180°around the axis of its skeletonized perforator vessel. 20 Propeller flaps have been described for soft tissue reconstruction throughout the body for defects of various etiology: tumors, chronic osteomyelitis, diabetic or chronic wounds, and traumas with deep structures exposure. 21,22 These flaps present many advantages and even more when raised in a free-style fashion.…”

“…However, vascular anomalies of the supraclavicular artery exist [8]. Some anatomical studies on how to improve the reliability of supraclavicular flaps have argued that anatomical knowledge of local perforasomes and vascular systems is very important because the supraclavicular flap is mostly based on relatively small and superficial perforators [9][10][11].…”
