Outcome after failure of allogeneic hematopoietic stem cell transplantation in children with acute leukemia: a study by the société Francophone de greffe de moelle et de thérapie cellulaire (SFGM-TC)

Abstract: Allogeneic hematopoietic stem cell transplantation (SCT) contributes to improved outcome in childhood acute leukemia (AL). However, therapeutic options are poorly defined in the case of post-transplantation relapse. We aimed to compare treatment strategies in 334 consecutive children with acute leukemia relapse or progression after SCT in a recent 10-year period. Data could be analyzed in 288 patients (157 ALL, 123 AML and 8 biphenotypic AL) with a median age of 8.16 years at transplantation. The median delay … Show more

“…In accordance with previously published case series and registry analysis in adults and children, our data indicate that length of remission after first transplantation is probably the most important outcome predictor for relapse after first allo‐SCT (Michallet et al , ; Bosi et al , ; Eapen et al , ; Kato et al , ; Bajwa et al , ; Naik et al , ; Menon et al , ; Roux et al , ).…”

“…Second transplantation has been shown to successfully sustain remission in 20‐35% of patients in several series in adults (Michallet et al , ; Bosi et al , ; Eapen et al , ) and up to 43·9% in children (Menon et al , ; Roux et al , ). Our data show that the 3‐year EFS was 33·6% in children undergoing salvage therapy with second SCT.…”

“…Post‐transplant leukemic relapse remains the most common cause of treatment failure, occurring in approximately 30%‐50% of children . Fewer than 20% of children who relapse after alloHCT achieve long‐term disease control . Effective strategies to minimize the risk of post‐transplant leukemic relapse are a priority.…”

“…After allogeneic hematopoietic stem cell transplantation (HSCT), relapse is a major cause of treatment failure for children with acute myeloid leukemia (AML) . Thus, the establishment of effective novel strategies to prevent relapse is a pressing issue.…”
