In this study, poly (trimethylene carbonate) (PTMC) with excellent biocompatibility was synthesized via ring-opening of TMC to prepare the Ciprofloxacin-loaded PTMC implants, and antibacterial effects in vitro or in vivo of the resulting implants were investigated to evaluate the potential for treating chronic osteomyelitis. The in vitro results showed the Ciprofloxacin-loaded PTMC implants could sustain release ciprofloxacin at a release amount of about 90 μg/d for 28 days and possessed excellent antibacterial effect, as evidenced by the smaller size of the antibacterial ring of 32.6 ± 0.64 mm and the biofilm inhibition of 60% after 28 days of release. The in vivo results showed that after 28 days of treatment, the body weight and the white blood cell counts of chronic-osteomyelitis-model rats in the treatment group reached 381.6 ± 16.8 g and (7.86 ± 0.91) ×109/L, respectively, returning to normal rapidly compared with the control and blank group, indicating the remarkable antibacterial effect of the Ciprofloxacin-loaded PTMC implants. X-ray images and HE staining results also confirmed that most of the proximal and middle parts of the tibia returned to typical structures and new and trabecular bone had been formed for the rats in the treatment group, and no inflammatory cells were found as compared to the control and blank groups, after 28 days of treatment. The significant lower number of colonies of (9.92 ± 1.56) × 10 CFU/g in the treatment group also suggests that the Ciprofloxacin-loaded PTMC implants achieve a practical antibacterial effect through a local application.

Osteomielitis merupakan komplikasi yang umum terjadi dan menjadi tantangan bagi ahli bedah. Pengobatan osteomielitis biasanya melalui pemberian secara sistemik, namun antibiotik ketika diberikan secara sistemik kadar antibiotik yang sampai pada jaringan rendah karena pemberian secara sistemik. Implant dengan lapisan antibakteri telah dikembangkan sebagai alternatif pengganti pengobatan antibiotik secara sestemik. Dari beberapa artikel ilmiah membuktikan bahwa tingkat kesembuhan keseluruhan pasien osteomielitis adalah 65,5% hingga 95% dengan terapi lokal menggunakan implant berlapis antibiotik. Pada penelitian ini dilakukan uji meta analisis dengan tujuan untuk mengetahui efektivitas implant berlapis antibiotik yang digunakan secara monoterapi atau terapi kombinasi pada pasien osteomielitis. Metode yang digunakan pada literature review ini adalah metode boolean. Sumber informasi didapatkan dari database Science Direct dan PubMed. Data yang digunakan untuk dilakukan meta analisis yaitu sebanyak 9 data yang memenuhi kriteria dilakukannya meta analisis. Pada meta analisis dilakukan analisa pada forrest plot untuk mengetahui efektifitas penggunaan implant berlapis antibiotik gentamisin sebagai monoterapi ataupun biterapi serta dilakukan analisa pada hasil funnel plot untuk mengetahui apakah terdapat bias publikasi pada hasil meta-analisis. Hasil forrest plot menunjukkan nilai random effect model (RE model) (0,86) dimana pada penelitian terdahulu mensyaratakan bahwa nilai size effect (0,88) yang paling mendekati nilai RE model dengan estimasi yang paling akurat. Pada funnel plot menunjukkan hasil yang simetris dimana nilai p-value pada egger’s test (0,667) dan nilai fail-safe N (10373) maka hasil meta analisis ini tidak memiliki makna bias. Sehingga dari hasil meta analisis tersebut telah dibuktikan bahwa antibiotik gentamisin efektif digunakan bersamaan dengan antibiotik vankomisin pada implant berlapis antibiotik pada pasien osteomielitis.

Background and Objectives: Cranial defects pose significant challenges in low and middle-income countries (LIMCs), necessitating innovative and cost-effective craniofacial reconstruction strategies. The purpose of this study was to present the Bosnia and Herzegovina model, showcasing the potential of a multidisciplinary team and 3D-based technologies, particularly PMMA implants, to address cranial defects in a resource-limited setting. Materials and Methods: An observational, non-experimental prospective investigation involved three cases of cranioplasty at the Department of Neurosurgery, Cantonal Hospital Zenica, Bosnia and Herzegovina, between 2019 and 2023. The technical process included 3D imaging and modeling with MIMICS software (version 10.01), 3D printing of the prototype, mold construction and intraoperative modification for precise implant fitting. Results: The Bosnia and Herzegovina model demonstrated successful outcomes in cranioplasty, with PMMA implants proving cost-effective and efficient in addressing cranial defects. Intraoperative modification contributed to reduced costs and potential complications, while the multidisciplinary approach and 3D-based technologies facilitated accurate reconstruction. Conclusions: The Bosnia and Herzegovina model showcases a cost-effective and efficient approach for craniofacial reconstruction in LIMICs. Collaborative efforts, 3D-based technologies, and PMMA implants contribute to successful outcomes. Further research is needed to validate sustained benefits and enhance craniofacial reconstruction strategies in resource-constrained settings.
