Outcome and attitudes toward home tracheostomy ventilation of consecutive patients: A 10-year experience

Abstract: TIPPV is well-received by the patients, is safe, and provides survival for long periods of time. Underlying conditions (COPD and ALS) might represent important prognostic factors for survival.

“…Our own patients reported diminished participation in social activities at least partially due to dependency on and appearance of tracheostomy. Several complications are related to tracheostomy tubes, such as obstruction, chronic aspiration, dysphagia, pulmonary infections, fistula formation or bleeding ulcerations, which all can have devastating or even life-threatening consequences (9)(10)(11)44). Therefore, decannulation after FT improves quality of life and prevents for potentially lethal complications.…”

“…Patients with severe facial injuries suffer from leakages of the oral cavity, trismus and absence of masseter muscles and thus are often unable to adequately ingest solid foods, liquids or even their medications (4). This ultimately leads to insertion of feeding tubes or gastrostomy, provoking significant discomfort, potential for malnutrition, infection, bleeding and possibly death (6)(7)(8)(9)(10). Even more hazardous to health are impairments of the upper airway.…”

“…Some patients may eventually reach a "locked in" state in which they cannot communicate at all, because there is also total paralysis of the extraocular muscles. When connected to tracheostomy tubes patients may survive for many years, with respiratory tract infections the most frequent cause of death (Bradley et al, 2002;Hayashi & Oppenheimer, 2003;Lo Coco et al, 2007;Marchese et al, 2008;Vianello et al, 2011).
