The aim of our study was to explore the risk factors for very preterm (gestation under 32 weeks) and moderate preterm birth (gestation weeks 32-36 6/7) in singleton pregnancies in a national retrospective cohort study. We also wanted to establish whether IVF/ICSI is an independent risk factor for preterm birth after adjusting for already known confounders. We used data for 267 718 singleton births from 2002-2015 from the National Perinatal Information System of Slovenia, containing data on woman, pregnancy, birth, the postpartum period, and the neonate for each mother–infant pair. Mode of conception, maternal age, education, BMI, parity, smoking, history of cervical excision procedure, history of hysteroscopic resection of uterine septum, presence of other congenital uterine malformations, bleeding in pregnancy, preeclampsia or HELLP and maternal heart, and pulmonary or renal illness were included in the analyses. Unadjusted OR for very preterm birth after IVF-ICSI was 2.8 and for moderate preterm birth was 1.7. After adjusting for known confounders, the OR was still significantly elevated (1.6 and 1.3, respectively). Risk factors for very preterm birth with OR higher than 2.4 were history of cervical excision procedure, resection of uterine septum, operation or having other congenital uterine malformations, and bleeding in pregnancy. Risk factors for very preterm birth with OR between 1.4 and 2.1 were age >35 years, being underweight or obese, not having professional education, smoking, first birth, preeclampsia/HELLP, and IVF/ICSI. Risk factors for moderate preterm birth with OR higher than 2.4 were history of cold knife conization and other congenital uterine malformations. We found that even after adjustment, IVF/ICSI represents a single risk factor for early and late preterm birth even after adjustment with other risks such as maternal age, smoking, or a history of invasive procedures for either cervical intraepithelial neoplasia or infertility treatment.

Objectives: This study aimed to analyze the clinical and imaging findings as well as the outcomes of patients with Mullerian duct anomalies. Materials and Methods: A retrospective analysis of 41 patients with Mullerian development anomalies treated in a tertiary care center in the past 9 years was done. The presenting symptoms, radiological findings, management, and the outcomes were evaluated. Results: According to the American Fertility Society's classification, 11 patients presented in Class I, 6 in Class II, and 24 in Class III of the classification. It was found that some of the defects such as the unicornuate uterus, a unicornuate uterus with noncommunicating rudimentary horn, and longitudinal vaginal septum were usually asymptomatic whereas disorders such as Mayer-Rokitansky-Küster-Hauser (MRKH), cervicovaginal atresia, and transverse vaginal septum presented with the absence of menarche, cyclical abdominal pain, and abdominal mass, respectively. Defects such as the bicornuate uterus, didelphys uterus, and septate uterus present with poor reproductive performance. Unicornuate uterus with communicating horn presented with rupture of the horn in the antenatal period, which was managed vigorously. Vaginoplasty with a skin graft and amnion graft had excellent results in MRKH syndrome. Patients with cervicovaginal atresia had a poor prognosis and ultimately required a hysterectomy. Hysteroscopic septal resection improved the reproductive performance in the patients with septate uterus. Conclusion: This study concluded that the management of uterine malformations is individualized depending on the symptoms and fertility concerns. Cervicovaginal atresia was associated with restenosis after surgery ultimately required a hysterectomy. MRKH had excellent results with McIndoe vaginoplasty. Optimal and timely management may lead to better outcomes.

Вступ. Частота невиношування вагітності складає 15-25% від усіх вагітностей, зростає з віком жінки і не має тенденції до зниження. Однією з причин невиношування вагітності є вроджені та набуті порушення анатомічної будови матки. Частота вроджених аномалій розвитку статевих органів у пацієнток зі звичним невиношуванням коливається в межах 3-16%.Мета. Оцінити діагностичну цінність та лікувальні можливості гістероскопії у пацієнток з аномаліями розвитку матки та репродуктивними втратами в анамнезі.Матеріали і методи. Нами проведена ретроспективна оцінка діагностичної цінності гістероскопії в диференційній діагностиці аномалій розвитку матки у 59 пацієнток з репродуктивними втратами в анамнезі та її лікувальних можливостей.Результати дослідження. Інформативність гістероскопії в діагностиці аномалій розвитку матки ретроспективно порівняли з результатами, отриманими під час гістеролапароскопії. 42 пацієнткам з інтраопераційно діагностованою матковою перегородкою відразу була проведена гістероскопічна метропластика. Повноту висічення внутрішньоматкової перегородки оцінювали порівнюючи дані 3D УЗД, проведеного до гістероскопічної метропластики та внаступному після оперативного втручання менструальному циклі. У 4,8% хворих необхідне було проведення повторного оперативного втручання, оскільки під час першої операції не вдалося одномоментно досягнути повного розсічення перегородки. Частота правильно класифікованих аномалій розвитку матки за допомогою гістероскопіїсклала 84,7%. Найбільші труднощі при оцінці гістероскопічної картини виникали у випадках диференціації дворогої матки та маткової перегородки. В подібних ситуаціях гістероскопічне обстеження повинно бути доповнене лапароскопією, яка дає змогу уникнути постановки хибного діагнозу. У випадку однорогої матки гістероскопічний діагноз співпадав із заключним у 100%. Однак навіть поєднане використання гістероскопії та лапароскопії спричиняє значні труднощі в диференціації підкласів даної аномалії, які базуються на наявності чи відсутності функціональної порожнини в рудиментарному розі. В подібних ситуаціях високоінформативним методом є 3D УЗД, за результатами якого в нашому дослідженні у 42,9% пацієнток з однорогою маткою мала місце функціонуюча порожнина в рудиментарному розі, що було показом до його видалення. У випадку подвоєної матки гістероскопічний діагноз співпадав із даними гістеролапароскопії у 60% випадків (р<0,05).Висновки. Гістероскопія повинна бути інтегрована в загальну діагностичну схему при обстеженні пацієнток з підозрою на Мюллерівські аномалії. Частота правильно класифікованих аномалій розвитку матки при самостійному виконанні гістероскопії складає 84,7%. Одночасне проведення лапароскопії та гістероскопії дає можливість в 100% випадків поставити точний діагноз перегородки матки та провести хірургічну корекцію даної аномалії зі збереженням цілісності стінки матки.Ключові слова: не виношування вагітності; гістероскопія; аномалії розвитку матки.
