Outcome ofin vitrofertilization following stimulation with highly purified hMG or recombinant FSH in downregulated women of advanced reproductive age: a prospective, randomized and controlled trial

Abstract: Following downregulated women of advanced reproductive age, superiority of HP-hMG over rFSH in live birth rate could not be concluded from this study, but noninferiority was established. Pharmacodynamic differences in follicular development, oocyte/embryo quality and endocrine response exist between HP-hMG and rFSH, which may be relevant to treatment outcome.

“…Among the reports included in the review, only one relatively small study (n = 127) provided live-birth data on patients who had not undergone IVF/ICSI prior to randomization,28 in women undergoing an IVF GnRH-agonist cycle. Therefore, we used data from a prospective cohort study conducted in Norway (n = 1,136)29 as the source for the base-case economic evaluation.…”

“…Of note, elevated progesterone had no impact on the oocyte and embryo quality thus suggesting that its likely detrimental effect relates to the endometrial receptivity [172]. This hypothesis has been corroborated by data from oocyte donation programs, which indicates that pregnancy rates of the recipients are not influenced by progesterone levels of the donors measured at the day of hCG administration [166,173,174].…”

“…In the event of COS, multiple follicular growth is achieved. Recombinant FSH has higher FSH bioactivity compared to urinary FSH and hMG, thus leading to the development of more follicles and granulosa cells [97,125,126,[166][167][168]. Furthermore, FSH induces up-regulation of LH receptors in granulosa cells, unlike hMG [22,96].…”

“…On the other hand, studies investigating the effects of folliclestimulating hormone (FSH) + human chorionic gonadotropin (HCG)-driven LH activity combination in HP-HMG and r-hFSHonly mostly involved general infertile women undergoing IVF using either agonist or antagonist protocol (Andersen et al, 2006;Esteves et al, 2009;Kilani et al, 2003;Lehert et al, 2010;Platteau et al, 2008;Shavit et al, 2016). To our knowledge, there are still a few studies that have investigated the effect of FSH + HCG-driven LH activity combination in HP-HMG in poor responders (Ji et al, 2020) or in advanced maternal age (≥35 years old) (Ye et al, 2012). Additionally, previous studies on the effect of LH activity supplementation on POR patients in IVF were mostly conducted in European region on the Caucasian population.…”
