Outcome of monochorionic twin pregnancies with abnormal umbilical artery Doppler between 16 and 20 weeks of gestation

Abstract: AREDF of the umbilical artery is uncommon in normal MC twin in mid-trimester. Once AREDF is present, the perinatal outcome is much worse. Doppler assessment of the umbilical artery should be considered in all MC twin pregnancies for risk assessment.

“…Furthermore, higher-order multiple pregnancies with MC components may benefit from multifetal pregnancy reduction (MFPR). While TTTS, TAPS and sFGR are diagnosed mostly after 16 weeks [1][2][3][4] , the other complications can be detected earlier, thus allowing early intervention; this is particularly desirable for TRAPS, as evidence has shown that 33% of pump twins die before 16-18 weeks 5 . Another potential advantage of early intervention is the technical ease of reduction when the fetus and umbilical vessels are still relatively small.…”

“…The prevalence of Abnormal UA in 84 MCDA twins between 16 and 20 weeks reported by Pan et al 16 was 25%; of these, 7.4% were uncomplicated cases and 56.7% were pathological cases, including TTTS or sIUGR. However, waveforms were not re‐evaluated afterwards, and the sampling point for UA Doppler was not mentioned.…”

“…Besides, the classification according to the end‐diastolic waveforms of UA Doppler in the smaller twin is useful to predict the prognosis of MCDA twins complicated with selective intrauterine growth restriction (sIUGR) 5–15 . Abnormal UA could be observed in more than half of the entire sIUGR cases 9,14,15 and in uncomplicated MCDA twins 16,17 . However, the prevalence of Abnormal UA among MCDA twins in the early second trimester remains unknown.…”

“…12 This retrospective study included 84 MCDA twin pregnancies, all of which had UA Doppler assessment during a routine mid-trimester scan. The pregnancies were then divided into groups based on the presence or absence of MCDA-specific complications (selective intrauterine growth restriction, growth discordance, TTTS) and/or abnormal UA Doppler studies at intake.…”
