Outcome of veno-venous extracorporeal membrane oxygenation use in acute respiratory distress syndrome after cardiac surgery with cardiopulmonary bypass

Abstract: VV-ECMO support can be a feasible rescue strategy for adult patients who develop refractory ARDS after a cardiac surgery. Additionally, the RESP score seems a valuable prognostic tool for post-ECMO survival outcome in this patient population as well.

“…The demographics of the patients, type of ECMO treatment used, and the risk factors or outcomes of colonic ischemia were not reported in either study. To date, 3 cases of ischemic colitis have been found in 2 of the studies that evaluated patients who received only VV-ECMO [ 5 , 6 ]. All of the patients were in their 70s and had respiratory failure due to various etiologies.…”

“…There is emerging but scarce evidence that ischemic colitis can occur in patients treated with VV-ECMO. To the best of our knowledge, only 3 such cases have been reported in the literature, all of whom subsequently died [ 5 , 6 ]. We present a rare case of a patient receiving VV-ECMO treatment who developed ischemic colitis with subsequent recovery with non-surgical management.…”

“…These three cases occurred over a 5-year period when approximately 500 TAAA repair cases were performed. Although ECMO for ARDS after TAAA repair has not been reported, early hypoxemia (within 24 hours after cardiac operation) is associated with 46% in-hospital mortality [6]. Hypoxemia adversely affects oxygen delivery to the brain, spinal cord, and other vital organs.…”

“…Most importantly, MV prior to initiation of ECMO is an important component in the calculation of Respiratory ECMO Survival Prediction (RESP) score. This score has been validated to assist prediction of survival for adult patients undergoing ECMO for respiratory failure [4, 32]. However, it is still difficult to determine a specific time point after which the initiation of ECMO can be considered futile.…”
