Abstract: To mitigate the negative impacts of HIV infection on pregnant women and their children, comprehensive services to prevent mother to child transmission are required. We report the outcomes and challenges of a comprehensive service to prevent mother to child transmission of HIV infection implemented from January to December 2007 in a referral hospital and its rollout clinics in rural Swaziland, Southern Africa. “…Scarce human resources and delivery facilities in rural areas were significant structural barrier reported by ORWs from the rural districts in our study. Similar problems have previously been reported elsewhere in India [22] and in Africa [23], and clearly pose a major challenge for ORWs to facilitate the service uptake of these women. These data helped us to develop standardized counseling scripts to address these specific challenges and would be available to the ORWs through their tablet computers.…”

“…It has been noted by Swaziland’s Ministry of Health that lack of timely CD4 assessments as well as limited clinic staff were challenges faced in antenatal health care facilities (Ministry of Health; Kingdom of Swaziland, 2010). Unpredictable episodes of ART supply shortages can possibly delay the availability of lab results and initiation of ART for infants (Bancheno, Mwanyumba, & Mareverwa, 2010). Poor treatment by healthcare workers, including patient-blaming, stigmatization, and rudeness in health facilities was a commonly stated barrier to child ART initiation, which was also discovered in other studies (Hodgson, et al, 2014; Yeap, et al, 2010).…”

“…The lack of paternal support, stigma, lack of testing or disclosure, gender iniquities and inequalities, intimate partner violence (IPV), lack of PMTCT information, clinic access and poor retention in care have been identified as major challenges to PMTCT effectiveness in South Africa’s rural areas [12–15]. The Mpumulanga province, the location of this trial, is a predominantly rural area with one of the lowest rates of PMTCT participation and one of the highest antenatal clinic (ANC) HIV prevalence rates (37%) in the country.…”
