Outcomes Following Modified Tympanomastoidectomy With Soft-wall Reconstruction

Abstract: MTMSWR is an option for restoring the posterior EAC wall following CWD surgery. Recidivism is comparable to canal wall up surgery, and hearing outcomes are generally better than CWD. Other advantages include rapid epithelialization, low rates of otorrhea, and restoration of a self-cleaning EAC regardless of retraction into the mastoid cavity.

“…Supplementing the recidivism rates, the frequency of CWD conversion at second look was utilized as a proxy to measure the relative extent of recidivistic disease. Although there was no statistically significant difference in CWD conversion between S-CWR (15.6%) and B-CWR (5.9%), our rates were similar to previously reported literature of S-CWR (12.8%) and B-CWR (2.5-4.0%) (13,15,16) Regarding audiometric outcomes in S-CWR versus B-CWR, there were no significant differences between the cohorts when measuring the change in PTA, ABG, SRT, and WRS pre-versus postoperatively. S-CWR demonstrated slight worsening in ABG by 1.55 dB on average following second-look ossiculoplasty, while B-CWR had slight improvement by 2.17 dB on average.…”

“…Overall recidivism rates in our cohorts were 21.2% in S-CWR and 21.1% in B-CWR. In only ME/mastoid recidivism, the rate for S-CWR was 17.3% and 18.4% in B-CWR, within the range of previously reported ME recidivism rates for CWR in the literature, ranging from 9.2% to 39% (8,10,11,13–16). Of note, our definition of recidivistic disease is more inclusive than that reported in other studies (e.g., including CT and/or MRI data).…”

“…Despite these factors, very promising results have been achieved. 11,12 In the present study, during the post-operative period, the patients achieved hearing results similar to those reported in the literature. One of the important issues in ear canal reconstruction is that detecting recurrent cholesteatoma is more difficult because a closed cavity has been created.…”
