Outcomes in 140 critically ill patients with status epilepticus

Abstract: Mortality in SE patients remains high and chiefly determined by seizure severity. Further studies are needed to evaluate the possible impact of early maximal anticonvulsant treatment on outcomes.

“…Third, to define status epilepticus, we used the recent operational definition requiring a seizure activity of more than 10 min and corresponding to impending status epilepticus, instead of 30 min (corresponding to established status epilepticus). Recent findings showed significantly higher mortality rates in patients with seizure activity lasting 30 min or more [34]. However, all five patients with ESE had epileptiform activity for 30 min at least.…”

“…In another study of 45 patients with ICH [90], subclinical seizures and non-convulsive status epilepticus were detected in 13% and 9% of the patients, respectively. Uninterrupted convulsive and non-convulsive seizures (status epilepticus) is in the ICU patients is associated with high mortality [91]. Therefore, a low threshold for obtaining EEG studies and use of anticonvulsants in patients with ICH may be advisable.…”

“…The 30-day mortality of patients with generalized convulsive SE ranges from 19 to 27 % [4][5][6][7][8], while more recent randomized controlled trials suggest that this rate might be lower (hospital discharge mortality of 9.4 %) with more inclusive definitions of SE [9]. Prolonged seizures are associated with higher mortality and worse clinical outcomes [7,8,[10][11][12].…”

“…Prolonged seizures are associated with higher mortality and worse clinical outcomes [7,8,[10][11][12]. Adverse effects of SE include both indirect systemic problems arising from the convulsive state (e.g., impaired ventilation, pulmonary aspiration, and metabolic aberrations) and direct neuronal cellular injury from excitotoxicity, causing both immediate neuronal loss and delayed programmed cell death.…”
