Outcomes of Adenotonsillectomy for Obstructive Sleep Apnea in Prader‐Willi Syndrome: Systematic Review and Meta‐analysis

Abstract: Objectives: Prader-Willi syndrome (PWS) increases the risk of obstructive sleep apnea (OSA) due to obesity, hypotonia, and abnormal ventilatory responses. We evaluated post-adenotonsillectomy complications, polysomnography changes, and quality of life in children with OSA and PWS. Study Design: Systematic review and meta-analysis. Methods: We conducted a systematic review and meta-analysis by searching PubMed, Embase, Cochrane, Web of Science, and Scopus. Two researchers independently reviewed studies about ad… Show more

“…In patients with PWS, SDB even contributes to increased mortality [ 29 ]. Nevertheless, studies have reported that patients with PWS undergoing adenotonsillectomy, the first-line treatment for OSAS in neurotypical children [ 12 ], have increased risk of developing postoperative complications and residual OSA [ 30 , 31 ]. As adenotonsillectomy is effective in some patients with PWS and OSAS, careful selection of appropriate surgical candidates is crucial.…”

“…PWS children may be helped by AT, with a meta-analysis of seven studies finding a mean post-operative improvement in AHI of 7.7 events•h −1 [75]. It is important to recognise that the most common complication of AT (in 14%) was velopharyngeal insufficiency [75].…”

“…However, it is estimated that up to 30% of children have persistent OSA after adenotonsillectomy. Risk is increased in children who have obesity or comorbid syndromes 44–47 . DISE is a flexible fiberoptic endoscopy performed while the patient is sedated and breathing spontaneously to determine sites of upper airway obstruction in a dynamic view.…”
