Outcomes of Infants Supported With Extracorporeal Membrane Oxygenation Using Centrifugal Versus Roller Pumps: An Analysis From the Extracorporeal Life Support Organization Registry

Abstract: Objectives: To determine if mortality differs between roller and centrifugal pumps utilized during extracorporeal membrane oxygenation (ECMO) in infants weighing less than 10kg. Design: Retrospective propensity matched cohort study Setting: All ECMO centers reporting to the Extracorporeal Life Support Organization (ELSO) Patients: All patients <10kg supported on ECMO during 2011-2016 within ELSO Registry Interventions: Centrifugal and roller pump recipients were propensity matched (1:1) based on predicted prob… Show more

“…It is therefore not surprising that bleeding related complications are common in cardiac ECMO patients with 28% of ECMO patients experiencing hemorrhagic complications. 36 Surgical site bleeding and other bleeding and clotting complications for the cohort are similar to those reported in the pediatric ELSO report. 4 TEG guided interventions could provide an avenue for appropriate targeted therapies and reduce these very significant complications.…”

“…Although most European centers use centrifugal pumps, differently to the USA, where the roller pump is common practice, this discrepancy in the management would not seem to impact on mortality even if O'Halloran et al demonstrated contrasting data in favor of the use of roller versus centrifugal pump [90].…”

“…As mentioned, we transitioned from the roller pump to the centrifugal pump in 2008, and this may define a major change in practice between the early compared with the later time epochs evaluated. However, several reports have suggested the centrifugal pump has been associated with increased hemolysis and organ injury (including neurologic complications) compared with the roller pump, 33,34 and thus, it is unlikely that this change can account for the improvement in outcomes observed over time. Further investigation regarding changes in injury rates over time, and the reasons behind them, are warranted.…”
