Outcomes of Pedal Artery Angioplasty Are Independent of the Severity of Inframalleolar Disease: A Subanalysis of the Multicenter RENDEZVOUS Registry

Abstract: Purpose: To examine the efficacy of pedal artery angioplasty (PAA) for chronic limb-threatening ischemia (CLTI) according to the severity of inframalleolar disease. Methods: In total, 257 consecutive CLTI patients (mean age 73.2 years; 175 men) with de novo infrapopliteal and inframalleolar artery disease were enrolled from the retrospective RENDEZVOUS registry. Inframalleolar artery disease was classified as moderate (Kawarada type 2, 144 patients) or severe (Kawarada type 3, 113 patients). PAA was performed … Show more

“…Adding pedal angioplasty has been shown to improve wound healing and shorten the time to heal but has not been shown to improve limb salvage or amputation-free survival. 109,110 The patency of pedal artery interventions has not been well defined. A disadvantage of this technique is the lack of a bailout strategy for severe dissection.…”

“…This has been confirmed in the RENDEZVOUS Registry, a recent multicenter retrospective analysis, which showed a higher and faster wound healing rate after pedal artery angioplasty, in particular among patients with moderate risk of delayed wound healing 15 . A subanalysis of this registry, focusing on the correlation between efficacy of pedal artery angioplasty and severity of pedal artery disease, showed better results in terms of limb salvage in treated versus untreated patients with Kawarada type 3 pedal arch 16 . Therefore, a significant effort should be made to bring in‐line flow directly to the forefoot, moving the focus to the pedal arteries and the pedal arch.…”

“…15 (b-d) the guidewire navigating through the deep branch up to the plantar arch and then retrograde through the lateral plantar artery reentering the common plantar artery, (e) the antegrade guidewire entering through the "gate" opened in the LPA ostium by the retrograde guidewire, and (f,g) the sequential angioplasty of LPA arch. 16 Therefore, a significant effort should be made to bring in-line flow directly to the forefoot, moving the focus to the pedal arteries and the pedal arch.…”

“…На тяжкість перебігу ГНПС впливає також вираженість діабетичної по-лінейропатії (ДПН). У цих пацієнтів з'являються трофічні виразки, які є джерелом інфікування глибоких клітковинних просторів стопи неклостридіальною інфекцією [13]. Ці особливості розвитку та перебігу ГНПС призводять до високої ампутації нижніх кінцівок у 8,3 % хворих на ЦД.…”
