Outcomes of surgical interventions for primary childhood glaucoma in Northern Tanzania

Abstract: Based on the success rates of this retrospective analysis, goniotomy or trabeculotomy for younger and trabeculectomy for older children can be recommended in our setting to reduce IOP. Late presentation in combination with advanced glaucomatous damage as well as erratic postoperative follow-up and treatment were likely factors that compromised overall success rates. More efforts are necessary to detect the blinding disease earlier and improve adherence to follow-up.

“…25 Some studies reported lower success rates of angle-based surgeries in PCG, ranging from 30% to 52% at 12 months, possibly because of the higher proportion of severe cases. 14,22 TSCPC demonstrated significantly lower success rates than other procedures (26.7% at 1 y and 20% at 3 y), which is slightly higher than a previously reported success rate of primary TSCPC in PCG patients (17% at 1 y). 14 TSCPC is typically reserved for refractory cases because of its unpredictability of IOP reduction effect and associated serious complications.…”

“…Goniotomy has been shown to have comparable success rates to trabeculotomy 14. The reported success rates for single goniotomy in PCG patients varied between 38% and 75% 14,23–25.…”

“…15 A comparable pre-operative intraocular pressure of 33.1 ± 10.2 mm Hg was observed in another study done in Northern Tanzania. 16 The procedure was found successful in 55(84.6%) out of 65 of our patients at the end of three months. A previous study on 196 eyes, operated for congenital glaucoma with goniotomy, reported a similar 80% success rate.…”

“…A study conducted in Northern Tanzania showed a success rate of 38% at 12 months follow up. 16 Another study showed a success rate of 50.6% in children undergoing goniotomy for primary paediatric glaucoma conducted by Hassan ein et al 19 The differences in the success rate of goniotomy in different studies may be due to factors like different follow-up period, patient compliance, socioeconomic status, the underlying cause for glaucoma and ethnicity.…”
