Outcomes of total hip arthroplasty, as a salvage procedure, following failed internal fixation of intracapsular fractures of the femoral neck

Abstract: The management of intracapsular femoral neck fractures in independently mobile patients remains controversial. Successful fixation obviates the limitations of arthroplasty for this group of patients, however with fixation failure rates as high as 30%, the outcome of revision surgery to salvage total hip replacement (THR) must be considered. We carried out this review to determine the outcome of salvage THR and how this compares to primary THR for fracture. We performed a PRISMA compliant systematic review usin… Show more

“…With the exception of very few patients with severe functional limitations, hemiarthroplasties should never be used for non-elderly patients, due to the risk of acetabular erosion [42] associated with high activity levels during many remaining life years. The salvage procedure has been shown to be associated with more complications than patients having primary THA, though this may be explained by selection bias [43].…”

“…[3][4][5][6][7][8] Moreover, treatment of proximal femoral fracture is associated with high complication rates (up to 35%) due to multifactorial and often patient-related factors as well as fracture etiology. 3,9,10 Osteosynthesis failure is considered a major complication, which necessitates changing or replacing the device to prevent further complications. In the latter one, total hip arthroplasty (THA) or hemiarthroplasty is necessary as a "salvage" procedure to maintain hip joint mobility and stability.…”

“…e incidence of infection and dislocation following THA for femoral neck fractures are probably the same as those in the general population, but we do not have the data to back this assumption. However, salvage THA after failed internal fixation of the neck fracture is associated with higher complication rates [30][31][32].…”
