Outpatient Readmission in Rheumatology: A Machine Learning Predictive Model of Patient’s Return to the Clinic

Abstract: Our objective is to develop and validate a predictive model based on the random forest algorithm to estimate the readmission risk to an outpatient rheumatology clinic after discharge. We included patients from the Hospital Clínico San Carlos rheumatology outpatient clinic, from 1 April 2007 to 30 November 2016, and followed-up until 30 November 2017. Only readmissions between 2 and 12 months after the discharge were analyzed. Discharge episodes were chronologically split into training, validation, and test dat… Show more

“…The advantages offered by the unique characteristics of the ANN model have been con rmed by statistical analyses. For example, using an ANN model can enable more appropriate and more accurate processing of inputs that are incomplete or inputs that introduce noise [9,27]. Another advantage is that linear and non-linear ANN models with good potential for use in large-scale medical databases can be constructed using data that are highly correlated but not normally distributed.…”

“…The study population was selected from the Hospital Clínico San Carlos Musculoskeletal Cohort (HCSC-MSKC), 14 a routine clinical practice cohort that includes subjects seen at the rheumatology outpatient clinic of our center whose clinical information and management was carried out using a departmental electronic health record (EHR). The HCSC-MSKC contains information from more than 35,000 patients attending our clinic from 1 April 2007 until 30 November 2017 and comprising more than 117,000 clinical visits.…”

“…Hospital readmission is a common occurrence that can impose a multitude of burdens such as increased costs and an increased risk of mortality. 1 Studies indicate that the percentage of patients readmitted to hospitals in 30 days following discharge may be as high as 20%. 2 Due to its cost and reoccurrence, in 2012 the Center for Medicare & Medicaid Services (CMS) instituted the Hospital Readmissions Reduction Program to reduce the frequency of rehospitalization by establishing financial penalties for hospitals with readmission rates deemed excessive.…”
