The use of videoconferencing in nursing for people in their homes

The aim of this literature review is to bring to the forefront knowledge about the suitability of videoconferencing in nursing for patients and their families living at home. A systematic literature review that included studies of videoconferencing in nursing care. Inclusion criteria were original studies relevant to concept of study. Computerized bibliographic databases (PubMed, CINAHL and PsycINFO) were searched from January 2008 to October 2016. In total, 325 articles were identified. Eight articles (1539 participants) met the inclusion criteria, and were finally included. The eight articles covered videoconferencing used in follow-up care for patients living in their homes after surgery, postpartum, chronic illnesses, families with premature infants, and children with cancer. A conventional content analysis was conducted. The results show that the success of videoconferencing depends on satisfying network access. Nurses as well as patients thought videoconferencing was similar to meeting in person. Videoconferencing seems to support person-centred nursing, promote selfmanagement and motivate patients and families to engage in their health. The use of videoconferencing requires further organization of the nursing practice. The study indicates that videoconferencing can be a suitable alternative to traditional nursing follow-up when network access and technology function properly and the nursing service is well organized. Videoconferencing seems to underpin person-centred nursing. Knowledge gaps identified, more research needed as bases for assessment of videoconferencing suitability in nursing for individual persons in different treatment situations.

Background Minimally invasive surgery (MIS) and enhanced recovery protocols (ERPs) have improved postoperative recovery and shortened length of hospital stay (LOS). Telemedicine technology has potential to improve outcomes and patient experience further. This study was designed to determine whether the combination of MIS, ERP and a structured telemedicine programme (TeleRecovery) could shorten total 30‐day LOS by 50 per cent. Methods This was a phase II prospective RCT at a large academic medical centre. Eligible patients aged 18–80 years undergoing minimally invasive colorectal resection using an ERP were randomized after surgery. The experimental arm (RecoverMI) included accelerated discharge on postoperative day (POD) 1 with or without evidence of bowel function and a televideoconference on POD 2. The control arm was standard postoperative care. The primary endpoint was total 30‐day LOS (postoperative stay plus readmission/emergency department/observation days). Secondary endpoints included patient‐reported outcomes measured by EQ‐5D‐5L™, Brief Pain Inventory (BPI) and a satisfaction questionnaire. Results Thirty patients were randomized after robotic (21 patients) or laparoscopic (9) colectomy, including 14 patients in the RecoverMI arm. Median 30‐day total LOS was 28·3 (i.q.r. 23·7–43·6) h in the RecoverMI arm and 51·5 (43·8–67·0) h in the control arm (P = 0·041). There were no differences in severe adverse events or EQ‐5D‐5L™ score between the study arms. The BPI revealed low pain scores regardless of treatment arm. Satisfaction was high in both arms. Conclusion In patients having surgery for colorectal neoplasms, the trimodal combination of MIS, ERP and TeleRecovery can reduce 30‐day LOS while preserving patients' quality of life and satisfaction. Background While an explosion in technological sophistication has revolutionised surgery within the operating theatre, delivery of surgical ward-based care has seen little innovation. Use of telepresence allowing offsite clinicians communicate with patients has been largely restricted to outpatient settings or use of complex, expensive and static devices. We designed a prospective study ascertaining feasibility and face validity of a remotely controlled mobile audiovisual drone (LUCY) to access inpatients. This device is, uniquely, lightweight, freely mobile and emulates 'human' interaction by swiveling and adjusting height to patients' eye-level. Methods Robot-assisted ward rounds (RASWRs) were conducted over 3 months. A remotely located consultant surgeon communicated with patients/bedside teams via encrypted audiovisual telepresence robot (DoubleRobotics, Burlingame, CA). Likert-scale satisfaction questionnaires, incorporating free-text sections for mixedmethods data collection, were disseminated to patient and staff volunteers following RASWRs. The same cohort completed a linked questionnaire following conventional (gold-standard) rounds, acting as a control group. Data were paired and non-parametric analysis was performed. Results RASWRs are feasible (>90% completed without technical difficulty). The RASWR (n = 52 observations) demonstrated face validity with strong correlations (r > 0.7; Spearman, p-value < 0.05) between robotic and conventional ward rounds among patients and staff on core themes, including dignity/confidentiality/communication/satisfaction with management plan. Patients (96.08%, n = 25) agreed RASWR were a satisfactory alternative when consultant physical presence was not possible. There was acceptance of nursing/non-consultant hospital doctor cohort [100% (n = 11) willing to regularly partake in RASWR]. Conclusion RASWRs receive high levels of patient and staff acceptance, and offer a valid alternative to conventional ward rounds when a consultant cannot be physically present.
