Background: Acinetobacter baumannii (A. baumannii) is a Gram-negative, aerobic coccobacillus. It causes life-threatening nosocomial infections, particularly in immunocompromised patients. Carbapenem-resistant A. baumannii are prevalent worldwide mostly caused by carbapenemase synthesis. Metallo β-lactamases (MBLs) include imipenemase (IMP), New Delhi metallo-β-lactamases (NDM) and Verona integrin metallo-β-lactamase (VIM). We aimed to investigate the prevalence of MBLs genes expression in carbapenem-resistant A. baumannii isolated from ICU patients as a reliable method for reducing the morbidity & mortality of these patients. Methods: Using conventional methods, 87 A. baumannii isolates were identified from 103 ICU patient specimens. Metallo β-lactamases were detected phenotypically in imipenem-resistant strains using a combined disc test (CDT). Real-time PCR was used to quantitate the expression of the blaIPM, blaNDM & blaVIM genes. Results: Imipenem resistance was identified in 82.8% of patients. Combined disc test was positive in 44.4% of imipenemresistant isolates. For metallo-lactamases gene expression, blaVIM had a higher median value than blaNDM and blaIPM (0.5, 0.1 and 0 respectively). Combined disc test was found to have a statistically significant relationship with both NDM and VIM gene expression, which was considerably higher in isolates with positive CDT. Both NDM and VIM gene expression had a statistically non-significant correlation with CDT value, but their expression had a statistically significant negative correlation with CD zone of inhibition value. Conclusion: The expression of blaVIM and blaNDM genes is directly correlated with the level of MBLs production and the level of these enzymes can be detected phenotypically depending on its negative correlation with the CD zone of inhibition diameter.

Healthcare-associated infections are an emerging cause of morbidity and mortality in COVID-19 intensive care units (ICUs) worldwide, especially those caused by multidrug-resistant (MDR) pathogens. The objectives of this study were to assess the incidence of bloodstream infections (BSIs) among critically ill COVID-19 patients and to analyze the characteristics of healthcare-associated BSIs due to MDR Acinetobacter baumannii in an COVID-19 ICU. A single-center retrospective study was conducted at a tertiary hospital during a 5-month period. The detection of carbapenemase genes was performed by PCR and genetic relatedness by pulsed-field gel electrophoresis (PFGE) and multilocus-sequence typing. A total of 193 episodes were registered in 176 COVID-19 ICU patients, with an incidence of 25/1000 patient-days at risk. A. baumannii was the most common etiological agent (40.3%), with a resistance to carbapenems of 100%. The blaOXA-23 gene was detected in ST2 isolates while the blaOXA-24 was ST636-specific. PFGE revealed a homogeneous genetic background of the isolates. The clonal spread of OXA-23-positive A. baumannii is responsible for the high prevalence of MDR A. baumannii BSIs in our COVID-19 ICU. Further surveillance of resistance trends and mechanisms is needed along with changes in behavior to improve the implementation of infection control and the rational use of antibiotics.

Carbapenem-resistant Acinetobacter baumannii is one of the major problems among hospitalized patients. The presence of multiple virulence factors results in bacteria persistence in the hospital environment. It facilitates bacterial transmission between patients, causing various types of infections, mostly ventilator-associated pneumonia and wound and bloodstream infections. A. baumannii has a variable number of resistance mechanisms, but the most commonly produced are carbapenem-hydrolyzing class D β-lactamases (CHDLs). In our study, the presence of blaOXA-23, blaOXA-40 and blaOXA-51 genes was investigated among 88 clinical isolates of A. baumannii, including 53 (60.2%) strains resistant to both carbapenems (meropenem and imipenem) and 35 (39.8%) strains susceptible to at least meropenem. Among these bacteria, all the isolates carried the blaOXA-51 gene. The blaOXA-23 and blaOXA-40 genes were detected in two (5.7%) and three (8.6%) strains, respectively. Among the OXA-23 carbapenemase-producing A. baumannii strains (n = 55), insertion sequences (ISAba1) were detected upstream of the blaOXA-23 gene in fifty-two (94.5%) carbapenem-resistant and two (3.6%) meropenem-susceptible isolates. A. baumannii clinical strains from Poland have a similar antimicrobial resistance profile as those worldwide, with the presence of ISAba1 among blaOXA-23-positive isolates also being quite common. Carbapenem resistance among A. baumannii strains is associated with the presence of CHDLs, especially when insertion sequences are present.
