Overview of bariatric and metabolic endoscopy interventions

Abstract: The rise of endoscopic techniques allowing weight loss offers an attractive alternative to surgical interventions in Western countries where the obesity epidemic has risen dramatically. Endoscopists are well positioned to manage obesity given their broad-based medical knowledge, understanding of gastrointestinal physiology, and training in endoscopic technique. The field of bariatric and metabolic endoscopy has permitted the development of several efficacious and safe technologies. This review focuses on techn… Show more

“…1 POSE uses an incisionless operating platform to create full-thickness plications in the gastric fundus, thereby reducing gastric volume, which, in turn, limits the quantity of food that can be consumed at a given time. 2 The procedure also causes antral dysmotility that prolongs satiety by inducing an earlier and longer feeling of gastric distention. In ESG, the endoscopist attempts to remodel the greater curvature of the stomach through the placement of full-thickness sutures.…”

“…Bulking agent development for obesity treatment is largely undeveloped and perhaps a rich opportunity for much-needed bariatric treatment innovation, especially in combination with the growing field of endoscopic bariatrics. Bulking agents have the potential to limit nutrient absorption by being a physical barrier to mucosal absorption in a manner similar to other endoscopic methods of decreasing absorption such as duodenal mucosal resurfacing and intestinal bypass liners [57 ▪ ]. Other considerations for the future use of bulking agents, in a similar manner to its use for fecal incontinence, could be to create a pseudo-ileocecal valve in patients with significant bowel resection to slow transit, decrease diarrhea, and perhaps improve absorption.…”

“…This classification is based on the methods registered in the USA by the FDA. On the other hand, there are numerous methods today that lack American FDA registration but are registered and approved within the European Union, or are approved in the context of experimental studies [18,19].…”
