Objective To investigate whether asymmetry exists in the left-and right-handed distribution of Design Retrospective evaluation of a case series. Population A total of 1054 consecutive women undergoing first-line surgical treatment for Methods Data were collected on indication for the intervention, age at surgery, parity and disease Main outcome measure Frequency of left-and right-sided ovarian endometriomas. Results Histologically confirmed endometriotic ovarian cysts were present in 561 women, which were on the left side in 255 instances, on the right in 148, and bilateral in 158. Normal probability plots were used to assess the homogeneity of a population of 327 luteal phases from apparently ovulatory menstrual cycles. The length of the luteal phase was defined as the interval (in days) following but not including, the luteinizing hormone peak, up to and including the day before onset of menstruation. A small sub-set of the population consisted of cycles with abnormally short luteal phases but the majority of the data followed a normal frequency distribution which gave a mean (+ SD) for normal luteal phase length of 14.13 (k 1.41) days. It was estimated that all cycles with a luteal phase < 9 days were abnormal, and that 74%, 22% and 2% respectively of cycles with luteal phases of 10, 11 and 12 days were also abnormal. The total incidence of short luteal phases defined as above was 5.2%.

The processes that precisely control the selection of ovulatory follicles from a growing cohort are poorly understood. This reduction in follicle number occurs through several phases of selection, consequently we limit the use of the term 'selection' to the first major reduction of growing follicles, at the pre-to early antral stage. The final process of selection, achieving the appropriate ovulatory number, is referred to as 'dominance'. We discuss possible mechanisms that could bring about these reductions and highlight intra-ovarian involvement, particularly via follicle-follicle interactions. Analogies are drawn between local ovarian events and processes commonly reported in the determination of cell fate in developmental biology. Two facets of intra-follicular interactions are proposed: initially that follicle-follicle interactions mediate early selection processes at the preantral stage, and later that during antral development dominant follicles directly affect the fate of the subordinate cohort members.
