Paediatric stone management: innovations and standards

Abstract: Purpose of reviewThe aim of this study was to discuss current trends and recent developments in the surgical management of paediatric urolithiasis. Recent findingsMedical expulsive therapy (MET) is considered as an option for the treatment of children with ureter stones. Extracorporeal shockwave lithotripsy (SWL) remains as a first option for majority of kidney stones and upper ureter stones. Advances in endourology have led to increased adoption of retrograde intrarenal surgeries (RIRS) and percutaneous nephr… Show more

“…The surgical options for pediatric urolithiasis are largely based on experiences and techniques used for the treatment of urolithiasis in adults [20]. The European Society for Paediatric Urology guidelines recommend PCNL as a mainline of treatment in renal stones larger than 2 cm in the renal pelvis or more than 1 cm in lower pole stones as well as staghorn stones [21].…”

“…The management of ureteral stones in children includes open surgery, ureteroscopy (URS), extracorporeal shock-wave lithotripsy (ESWL), and percutaneous nephrolithotomy (PCNL). However, these procedures are costly and invasive and require anesthesia [ 6 ]. In recent years, MET (medical expulsive therapy) including alpha antagonists, steroids, and calcium channel blockers, has been used to enhance the distal ureteral stone expulsion rate in adults [ 6 ].…”

“…However, these procedures are costly and invasive and require anesthesia [ 6 ]. In recent years, MET (medical expulsive therapy) including alpha antagonists, steroids, and calcium channel blockers, has been used to enhance the distal ureteral stone expulsion rate in adults [ 6 ]. Regarding the high risk of stone recurrence in patients, invasive treatments need repetition, and successful observations will be associated with less of a risk for urinary system manipulation and anesthesia.…”
