Pancreas divisum as a predisposing factor for chronic and recurrent idiopathic pancreatitis: initial in vivo survey

Abstract: This is the first study to describe the prevalence of PD and PD subtypes in a community population and their association with idiopathic pancreatitis in vivo based on the findings of non-invasive MR and with minimal selection bias. It is concluded that PD should be considered a predisposing factor for chronic and recurrent pancreatitis.

“…PD was present in 35% of patients with pancreatitis versus 2.6% in the general community and had an odds ratio of 23.4 for inducing pancreatitis. 44 However, recent studies have shown an association of genetic mutations and PD in patients with IRAP. We have shown that SPINK1 (pancreatic secretory trypsin inhibitor) gene mutations were present in 40% of patients with IRAP and PD.…”

“…MRCP allows for non-invasive assessment of the distal CBD and pancreatic duct (PD) to detect for ductal dilatation, IDE and anatomical variants such as pancreas divisum which has a prevalence of between 1.5% and 6% 9 (in which case attempts at post resection PD stenting may prove frustratingly futile!). Its accuracy is reported at 81% 3 , though this is in the hands of expert pancreatico-biliary radiologists.…”

“…In pancreas divisum normal fusion fails to occur: the dorsal duct drains most of the pancreas through the minor papilla and the ventral duct drains only the head of the pancreas through the major papilla. Pancreas divisum is diagnosed by pancreatography either by ERCP or MRCP [18,33,34].…”
