Pancreatic carcinoma associated with marked eosinophilia: A case report

Abstract: A case of pancreatic carcinoma associated with marked eosinophilia is reported. A 71‐yr‐old man was admitted to hospital because of melena and abdominal pain. The systematic examinations revealed pancreatic adenocarcinoma with multiple metastases (rectum, lung and brain). The leukocyte count was gradually increased and reached up to 81.7 times 109/1, of which 54% consisted of eosinophils. Colony‐stimulating factor (CSF) was detected both in the patient's serum and in the tumor extracts by a normal human bone m… Show more

“…Az első elérhető közlemény 1987-ből származik egy 71 éves férfi betegről, akinél a melaena és hasi fájdalom miatt végzett vizsgálatok hasnyálmirigy-adenocarcinomát igazoltak végbél-, tüdő-és agyi áttétekkel. Esetében a maximális fehérvérsejtszám 81,7 G/l volt, melynek 54%-át adta eosinophil granulocyta [7]. A második esetet 2008-ban Ashdmir és mtsai közölték.…”

“…The diagnosis of eosinophilic pancreatitis is important and difficult [20] because it can mimic a pancreatic neoplasm [21] and is associated with eosinophilic gastroenteritis [10,[22][23][24][25][26][27][28][29] and hypereosinophilic syndrome [30]. Eosinophil-associated pancreatic disorders mainly include eosinophilic pancreatitis [31,32], pancreatic cancer [33], and autoimmune pancreatitis [34].…”

“…The real diagnosis of EP is very important and is made after the exclusion of parasitic infections and other gastrointestinal symptoms that exhibit marked eosinophilic infiltration in pathological samples [25,35]. However, some cases of pancreatic cancer have been associated with marked eosinophilia [21,33]; therefore, due to several similar clinical symptoms, features, and imaging results, sometimes EP is misdiagnosed as pancreatic cancer and is often treated surgically [14,17,36,37]. There is evidence indicating that eosinophilic pancreatitis is frequently diagnosed only after "false positive" pancreatic resection for suspected pancreatic tumour, and eosinophilic pancreatitis can mimic a pancreatic neoplasm [10,11,37].…”

“…In humans is still difficult to establish the pathogenesis of eosinophilic pancreatitis, but frequent causes for different grades of eosinophilic pancreatic infiltration include food allergy [13,54], hypersensitivity response to medications such as carbamazepine [55,56], parasitic infections [57], alcoholic [39] and non-alcoholic chronic pancreatitis, especially with pseudocyst formation [58,59] and pancreatic cancer [27,60,61]. Less frequent pathologies such as pancreatic allograft rejection, inflammatory myofibroblastic tumor [24] and histiocytosis X were also reported [26,27,62].…”
