Pancreatic pseudocyst: combined single incision laparoscopic cystogastrostomy and cholecystectomy in a resource poor setting

Abstract: Laparoscopic cystogastrostomy is a well-accepted minimally invasive modality to treat pancreatic pseudocysts. There has been one prior report of cystogastrostomy via single incision laparoscopic surgery (SILS) in which specialized instrumentation and access platforms were used.We report the challenges encountered in a low resource setting with the SILS approach to drainage using only standard laparoscopic instruments. To the best of our knowledge this is the second report of SILS cystogastrostomy and the first… Show more

“…To date there have only been one published case report of totally LPD[ 59 ] and three (un-published) laparoscopic-assisted PDs performed in Trinidad & Tobago. Otherwise, the reports of minimally invasive pancreatic surgery in the Anglophone Caribbean have been limited to three small series of laparoscopic distal pancreatectomies[ 57 , 60 , 61 ] and two reports of laparoscopic cysto-gastrostomy[ 62 , 63 ]. Up to the year 2021, there were no surgical robots in any English-speaking Caribbean country.…”

“…The advantages of a minimally-invasive approach for the treatment of pancreatic pseudocysts and WOPN are well known[8,9]. In the last decades different minimally invasive approaches have been described in literature, including endoscopic[10], laparoscopic[11,12], SILS[13] and robotic approach[5-7]. Several cases of laparoscopic drainage of pancreatic pseudocysts by cystogastrostomy have been described since the late 90s by Smadja et al[14].…”

“…Oncologic surgery in the Caribbean has evolved over the past decade, with increasing reports of advanced minimally invasive operations being reported in the region [1][2][3]. However, as it relates to pancreatic surgery, the minimally invasive approach has been limited to operations for pancreatic pseudocysts and distal pancreatectomies, with little application to periampullary lesions [4][5][6]. This is because a totally laparoscopic Whipple's operation is a technically demanding and time-consuming operation.…”
