Pancreato-biliary endoscopic ultrasound in opium addicts presenting with abdominal pain

Abstract: Objective:Asymptomatic dilatation of bile duct and symptomatic sphincter of Oddi dysfunction have been reported in opium addicts. Except one case report, there is no report in the literature on endoscopic ultrasound (EUS) study of pancreato-biliary system in opium addicts. The aim of the present study was to report the EUS features of pancreato-biliary system in opium addicts presenting with abdominal pain.Patients and Methods:A total of 15 opium addicts presenting with upper abdominal pain and dilated common … Show more

“…We have shown that in comparison with control patients, patients with opium addiction exhibit larger ampulla of Vater, a finding compatible with that reported by Sharma et al 10 However, the size of the ampulla of Vater in Sharma and colleagues’ study was greater than that reported in our study (71.9 vs 39.1 mm 2 ). This difference might, at least in part, be due to the longer duration of opium addiction in their patient population.…”

“…Although in Sharma and colleagues’ study, 10 CBD dilatation was noted in all 15 cases, PD dilation in the pancreatic body was detected in only two cases. In contradistinction, amongst our 56 cases, all patients exhibited both dilated CBDs and PDs.…”

“…Other studies have shown a significant relationship between CBD diameter and opium addiction. 13 , 16 The present study found a relationship between CBD stone size and addiction where the stones of the addicted patients were significantly bigger. It mainly happens because the opium causes the bile ducts to get dilated, resulting in the stones with a larger diameter blocking the bile flow, thereby leading to the choledocholithiasis signs in the patients.…”

“…In a recent study, opium addicts, symptomatic for abdominal pain were subjected to EUS. The authors observed CBD dilatation, especially in the extra hepatic tract, in all 15 patients included and increased surface area of Vater's papilla in 12 of them, after a mean of 20 years of opium addiction [21] . Pathologic conditions are also able to induce isolated bile duct dilatations with non-specific symptoms or biochemical abnormalities.…”
