Paradoxical Response to Chemotherapy in Neurotuberculosis

Abstract: Objective: Our objective was to investigate the paradoxical response and the factors responsible for such an uncommon (but known) response to chemotherapy in neurotuberculosis. Material and Methods: Ten children with intracranial tuberculomas/neurotuberculosis were retrospectively studied, who were on regular antituberculous chemotherapy. Any deterioration of the neurological condition, increase in the size of the lesion or appearance of new lesions was studied clinically and radiologically during the follow-u… Show more

“…(4,5,6,7) Most cases had history of TBM and can occur as paradoxical response to ATT even up to 1 year of chemotherapy. (8) The pathophysiology of the paradoxical response is thought to be a result of an immunological reaction. MacDonell et al reviewed a total of 18 cases of intra-medullary tuberculomas of the spinal cord reported over a period of 30 years.…”

“…This can occur spontaneously, as the immune system reacts to mycobacteria being killed by active drugs, or it can be seen in the setting of HIV infection, when the immune system is reconstituted by the use of antiretroviral therapy. In one case series of HIVnegative children, this paradoxical reaction manifested as the appearance of new granulomas and tuberculous abscesses, new hydrocephalus, or an increase in size of preexisting granulomas, and was observed from 3 weeks up to 1 year after the start of antituberculous therapy [34].…”
