Paraduodenal hernias in children: Etiology, treatment, and outcomes of a rare but real cause of bowel obstruction

Abstract: HighlightsParaduodenal Hernia has rarely been reported in the pediatric age group.Paraduodenal Hernia is a rare cause of intestinal obstruction and is often misdiagnosed.CT-scan is the gold standard mean for diagnosis in most cases.Paraduodenal Hernia must be kept in mind as a possible cause of intestinal obstruction.

“…1 Paraduodenal hernias rarely cause intestinal obstruction. 2 A right paraduodenal hernia is congenital and occurs when the small bowel herniates through a defect in the first portion of the jejunal mesentery (Figure 1). A left paraduodenal hernia is a congenital defect, where the small bowel loops prolapse through and into the left portion of the transverse mesocolon (Figure 2).…”

“…4 It is very rare for patients to be diagnosed preoperatively, however, diagnosis can be done with ultrasound and abdominal CT. Due to the risk of bowel obstruction, the mortality and morbidity rates associated with a paraduodenal hernia are high. 2 Treatment associated with this hernia include exploratory laparotomy, repair and reduction of hernia, and laparoscopic repair of the hernia. 5 Here we present the case of a 5-year-old female found to have a fatal bowel obstruction due to a paraduodenal hernia.…”

“…Common etiological causes of intestinal obstruction include adhesions, obstructed hernias, intussusception especially in infants, volvulus and round worms most common in older children 5 . Intestinal obstruction is a life threatening emergency if not diagnosed and treated in time, causes compromise of blood supply to the intestines leading to necrosis, gut perforation and eventually sepsis and death of the patient.…”

“…A left PDH results from failure of the mesocolon to fuse with the parietal peritoneum, and a right PDH occurs from incomplete rotation of the midgut. The resultant paraduodenal fossa allows small intestinal herniation with possible bowel entrapment and ischemia ( 3 , 4 ).…”
