Gallbladder paraganglioma (GP) is a rare tumor, with only 12 cases reported in the literature to date. Due to its rarity, clinical information of GP is insufficient. We present a case of GP in a 48-year-old female along with a literature review of all GP cases described to date. A 48-year-old female presented with intermittent right upper abdominal pain. Preoperative imaging revealed a hematoma in the gallbladder lumen without any definite etiology. Laparoscopic cholecystectomy was performed. Gross examination of the gallbladder revealed multiple small stones and a large hematoma as well as a 1.6-cm-sized polypoid mass at the gallbladder fundus. Microscopic study of the polypoid mass showed a zellballen appearance. Immunohistochemical analysis revealed that the mass was positive for synaptophysin, CD56, and chromogranin, suggesting GP. GP is difficult to diagnose because of non-specific clinical findings. Almost all GP cases are diagnosed based on histologic findings after cholecystectomy. Simple cholecystectomy was performed as a treatment in all reported cases of GP, including our case. There was no postoperative tumor recurrence or metastasis after surgery.

A 34-years-old married middle-class housewife without diabetes, hypertension or betel-nut- chewing hailing from Manikganj, Dhaka; came with the outside diagnosis as neuroendocrine tumor of the gallbladder to the Oncology Department of Khwaja Yunus Ali Medical College and Hospital (KYAMCH). She was admitted in the hospital with complaints of upper abdominal pain, abdominal bloating, weakness, anorexia, nausea and heartburn while taking fatty food. According to her Ultrasonography (USG) of the abdomen, a soft tissue mass was found in the gallbladder with unremarkable routine blood examination including tumor markers. She underwent open cholecystectomy with the suspicion of cancer. Grossly, gallbladder was measures 7.0x 5.0 cm with 0.3 cm wall thickness and on opening a polypoid nodule is found. Before treatment on request from the Oncology Department of KYAMCH, all slides were reviewed and a histopathological diagnosis of paraganglioma was made on the basis of organoid, nested or lobules (Zellballen pattern) of tumor cells in the lamina propria surrounded by a prominent fibrovascular stroma. Atypical mitoses, tumor necrosis, lymphovascular or perineural invasion was not found. The IHC reveals diffuse and strong positive reactions to NSE and CD56 for chief cells and strong positive reaction to S-100 protein for sustentacular cells. The epithelial tumor was ruled out by CK negative reaction. Therefore, the diagnosis of paraganglioma of the gallbladder was established. Because of the positive reaction to sustentacular cells, the neuroendocrine tumor has been excluded. To our knowledge, this type of case has not been reported in our country. The gallbladder is involved by a wide variety of non-epithelial tumors, apart from mesenchymal neoplasms. Visceral malignant melanoma develops in the biliary tract, including the gallbladder. As both primary and metastatic gallbladder melanomas may show similar of the same growth patterns, the differential diagnosis between these two conditions is often difficult. Primary gallbladder melanoma is more likely if the tumor shows a polypoid or lobulated and in particular papillary growth pattern, is a solitary lesion, and is associated with junctional activity in the adjacent mucosa. In contrast, metastatic lesions are more commonly multiple, flat, and without junctional changes. Apart from neuroendocrine tumors, other neuroectodermal neoplasms can develop as primary tumors in the gallbladder. Rare primary neoplasms in this organ include germ cell tumors, rhabdoid tumors, perivascular epithelioid cell tumors, and hemolymphatic neoplasms. Apart from malignant melanoma, numerous other malignancies can metastasize to the gallbladder, renal cell carcinoma being a particularly common metastasizing tumor. Malignant Melanoma of the Gallbladder Pathogenic Pathways Few melanocytes are present in the normal gallbladder mucosa, and it has been suggested that malignant melanomas take their origin from these cells that have spread from the neural crest to the gallbladder during oncogenesis. Primitive Neuroectodermal Tumor (PNET) of the Gallbladder Differential Diagnosis Differential diagnosis on conventionally stained sections mainly includes other small cell tumors occurring in the gallbladder, e.g., small cell carcinoma or lymphoma. Differential Diagnosis Gallbladder paraganglioma may occur as a component of multiple paragangliomas, involving the common bile duct and the liver in addition to the gallbladder (Ferrell et al. 1990).
