The skin plays a critical role in the detection of internal malignances. Cutaneous signs of these disorders afford clinicians opportunities for early diagnosis and treatment. We aim to succinctly review the recognition, diagnosis, and treatment of selected cutaneous paraneoplastic diseases. Skin disorders that may be associated with paraneoplastic syndromes include: cutaneous metastases, tripe palms, Sweet's syndrome, glucagonoma, Paget's disease and extramammary Paget's disease, acanthosis nigricans, Birt-Hogg-Dube syndrome, basal cell nevus syndrome, Bazex syndrome (acrokeratosis paraneoplastica), carcinoid syndrome, Cowden's disease(multiple hamartoma syndrome), dermatomyositis, erythema gyratum repens, ichthyosis aquisita, von Recklinghausen's disease, pityriasis rotunda, pyoderma gangrenosum, Quincke's edema (angioedema and paraneoplastic uricaria), paraneoplastic pemphigus, Degos' disease, superior vena cava syndrome, Werner's syndrome, diffuse normolipemic plane xanthomas, and yellow nail syndrome. Treatment for these disorders depends on the nature and anatomic distribution of the primary neoplastic process.

Acrokeratosis paraneoplastica Bazex (Bazex syndrome) is a rare paraneoplastic skin disease defined by erythematous, violaceous, scaly plaques on the hands and feet and on other acral locations such as nose and ears. Bazex syndrome is linked to a variety of underlying malignancies. Usually the skin lesions develop prior to the diagnosis of an internal malignant neoplasm with spontaneous remission after tumour removal. The objective of this study was to review the so far reported risk factors, diagnostic work-up, prognosis and treatment options for Bazex syndrome in a systematic manner. This systematic review is based on a search in MEDLINE, EMBASE and Cochrane Central Register for English and German articles from 1990 to 2015. Evidence on the diagnosis and treatment of Bazex syndrome is limited predominately to case reports or to small case series. There are no randomized controlled trials. A number of underlying tumour entities, predominately oropharyngeal neoplasms and tumours of the gastroenterological tract, but other malignancies were reported. Treatment modalities including topical and systemic corticosteroids, salicylic acid, topical vitamin D analogues, etretinate and PUVA therapy are often ineffective. Due to the small number of patients and the frequent misdiagnosis of this clinical entity, the aim of this systematic review was to call attention to this rare condition and to help clinicians to diagnose and treat Bazex syndrome effectively. Because of the good prognosis of the skin lesions and the tendency to resolve spontaneously if the underlying tumour is treated early, the differential diagnosis of Bazex syndrome should be taken into consideration when dealing with atypical psoriasiform cutaneous lesions. An early diagnosis may improve the patient's prognosis substantially.

AnamneseDer 61-jährige Patient in gutem Allgemein-und Ernährungszustand berichtete über Hautveränderungen im Bereich der Akren sowie über Nagelveränderungen an beiden Händen, die ein Jahr zuvor aufgetreten und seitdem progredient seien. Die bisherigen Therapieversuche mit Isosorbiddinitrat-Salbe und Mometason-Creme führten nicht zur Verbesserung des Lokalbefundes. Nebenbefundlich bestand ein chronischer Nikotinabusus (40 pack years) sowie Hämorrhoiden II°. Fieber, Nachtschweiß, Gewichtsverlust, Müdigkeit, Schmerzen, Appetitlosigkeit bestanden nicht. Eigen-und Familienanamnese bezüglich dermatologischer Erkrankungen war leer. Klinischer BefundBei der klinischen Untersuchung zeigten sich akrolokalisierte Erytheme mit pityriasiformer Schuppung an Fingerspitzen, Nasenspitze und Ohrhelices. Nagelveränderungen zeigten sich an beiden Händen und Füßen mit Onychodystrophie, subungualen Hyperkeratosen und Onycholysen (Abbildung 1, 2). Der Lymphknotenstatus war palpatorisch unauffällig. Das restliche Integument sowie die einsehbare Schleimhaut waren ebenfalls unauffällig. Histologischer BefundRegelrecht geschichtetes, hyperkeratotisch verhornendes Epithel, mit Parakeratoseeinschlüssen. Subepidermal ein Plötzlich aufgetretene, akrolokalisierte Erytheme mit Hyperkeratosen und pityriasiformer Schuppung an Fußsohlen, Fingerspitzen, Nase und Ohrhelices Sudden onset of acral erythema with hyperkeratosis and pityriasiform scales on soles, fi ngertips, nose and ear helices Abbildung 1 Klinisches Bild vor Therapiebeginn.
