Paraneoplastic Guillain-Barré Syndrome in Small Cell Lung Cancer

Abstract: Guillain-Barré syndrome (GBS) is defined as an acute, autoimmune polyradiculoneuropathy. It is a rare disease that occurs at a rate of 1.11 cases per 100,000 person-years. However, once infected, up to 20% of patients develop severe disability, and approximately 5% die. There have been reports of GBS in different cancers. Among them, there are 6 previous reports of GBS in small cell lung cancer. Here, we report a case of a 52-year-old man who was diagnosed with GBS in the setting of small cell lung cancer with… Show more

“…It has been hypothesized that tumors could produce antigens that cause autoimmunity against the peripheral nerves; this is seen in notable paraneoplastic syndromes that affect the nervous system [ 15 ]. Another hypothesis suggests that the relative immunosuppression related to cancer or its treatment could lead to GBS through immune regulation defects [ 15 ]. The underpinnings of the possible immune-mediated mechanisms remain unclear at the present time.…”

“…There are currently nine reported cases involving lung cancer [ 3 - 11 ]. Of the reported cases, seven involve small cell lung cancer [ 3 , 5 - 9 , 11 ] and two involve adenocarcinoma of the lung [ 4 , 10 ]. There is only one other documented case involving squamous cell, however the primary tumor originated from the mandible [ 20 ].…”

“…Based on the expected number of malignancies, the calculated odds ratio of GBS incidence was 2.4, suggesting a possible correlation between some cases of GBS and cancer ( 14 ). To our knowledge, 12 cases with GBS or GB-like syndromes associated with lung cancer have been reported ( 11 , 15 - 25 ). Interestingly, anti-ganglioside antibody was detected in only one case ( 11 ).…”
