Paraneoplastic syndromes of the peripheral nerves

Abstract: There is increasing recognition of an extensive variety of paraneoplastic disorders of the peripheral nerves. In many of these disorders onconeuronal antibodies are absent. Whole body fluorodeoxyglucose positron emission tomography scanning helps uncover the associated tumor, and recently proposed criteria may assist in the diagnosis. In many instances, prompt treatment of the tumor and immunotherapy result in symptom stabilization or neurologic improvement.

“…Examples include anti-Hu associated with small cell lung cancer (SCLC) and neuroblastoma, anti-Yo with breast and ovarian cancer, anti-Tr with Hodgkin’s lymphoma, anti-Ma with testicular cancer, anti-Ri with breast cancer and neuroblastoma, anti-amphiphysin with breast cancer, Lambert-Eaton myasthenic syndrome antibody with SCLC, recoverin with SCLC and so on [7] (Table 1, [21,22]). However, these antibodies might not be positive in all cases of paraneoplastic neurological syndromes as seen in our patient [23]. …”

“…Neuronal antigens expressed by the tumor stimulate an immune response characterized by T cells, antibodies, or both, which attack not only the tumor but also the neural tissue [8]. Although these scenarios could be expected in lymphomas, nervous system involvement is not commonly reported in reviews of SS [1,2].…”

“…Contrastingly, Hodgkin’s lymphoma (HL) usually damages peripheral nerves by an inflammatory process and may be associated with Guillain–Barre syndrome or chronic inflammatory demyelinating polyneuropathy 5. Paraneoplastic neuropathies have a heterogeneous phenotype and the classical sensory ganglionopathy may be absent 6. HL has been reported in association with a sensory neuropathy and various paraneoplastic antibodies, akin to anti-Hu.…”
