Parents' early healthcare transition experiences with preterm and acutely ill infants: a scoping review

Abstract: Opportunities to improve parents' early transition experiences include enhanced engagement, communication, information-sharing and shared decision-making between health care providers and parents. Future areas of research should focus on early transition interventions to advance parent capacity, confidence and closeness as the primary nurturer.

“…There are, for example, several US studies about neonatal transfer from the 1990s (Kenner et al., ; Kolotylo, Parker, & Chapman, ; Kuhnly & Freston, ; Page & Lunyk‐Child, ; Slattery, Flanagan, Cronenwett, Meade, & Chase, ). During that decade, US and Canadian researchers seemed especially interested in parents’ perceptions of neonatal back‐transfer (Ballantyne et al., ). Kolotylo et al.…”

“…The parents views on the neonatal environment as having a significant impact on their experience were consistent with various authors (Cleveland, ; Hall & Brinchmann, ; Williams et al, ); certainly, providing an environment that encourages proximity and avoids physical barriers was seen to be a source of comfort, in line with the term “emotional closeness” stated by Flacking, Thomson, and Axelin (). For transitions within neonatal care, parents voiced mixed emotions commonplace through the trajectory of neonatal care (Ballantyne et al, ), and regarding going home, persistent accounts of emotional stress have been reported in relation to both the very premature (Petty, Whiting, Green, & Fowler, ) and the late premature baby (Premji et al, ).…”

“…Raising staff awareness of these significant moments in the parent's journey is required so that they can focus on opportunities to improve parents’ early transition experiences. As Ballantyne et al () acknowledge, this includes ensuring staff–parent engagement, communication, information‐sharing and decision‐making is shared between both parties, continuing through the whole “illness‐health trajectory” (p. 783). In addition, these authors highlight the importance of a focus on early transition strategies to nurture parent confidence, competence in the parenting role and ongoing closeness to their babies.…”
