Parkinson risk in idiopathic REM sleep behavior disorder

Abstract: Objective: To precisely delineate clinical risk factors for conversion from idiopathic REM sleep behavior disorder (RBD) to Parkinson disease, dementia with Lewy bodies, and multiple system atrophy, in order to enable practical planning and stratification of neuroprotective trials against neurodegenerative synucleinopathy. Methods: In a 10-year prospective cohort, we tested prodromal Parkinson disease markers in 89 patients with idiopathic RBD. With Kaplan-Meier analysis, we calculated risk of neurodegenerative… Show more

“…Indeed, the percentage of iRBD subjects with DNH loss in this small explorative series is strikingly similar to the percentages of converters from iRBD to a neurodegenerative disorder observed after a 7.5-year follow-up in a recent prospective study. 3 Nevertheless, our study has some limitations: First, our iRBD sample was small and larger cohorts should be studied to confirm and consolidate our findings. In particular, with only 3 iRBD subjects with normal DNH findings, we are not able to assess associations of DNH status with other potential prodromal PD features, such is hyposmia.…”

“…RBD is the strongest known risk factor for neurodegenerative synucleinopathies, including Parkinson's disease (PD), dementia with Lewy bodies (DLB), and multiple system atrophy. Long-term studies have shown that the majority of persons with onset of idiopathic RBD in mid-life are actually in prodromal stages of neurodegeneration [3][4][5].…”

“…Regardless, we captured a large amount of data with 353 days and nights of observation from just 7 individuals. By contrast, some of the largest observational studies of sleep disorders in PD have included 100–200 nights of observation across the same number of participants [17, 18]. Finally, the need for some participants to carry an accelerometer potentially introduced observational bias.…”
