Breast cancer survivors (BCS) are often characterized by decreased pressure pain thresholds (PPT), range of motion (ROM) and strength in and around the shoulder affected by the treatment. This intra-rater reliability study was to establish the relative and absolute reliability of PPT's, active ROM and maximal isokinetic muscle strength (MIMS) of the affected shoulder in BCS with persistent pain after treatment. Methods Twenty-one BCS participated in the study. The PPTs of 17 locations and pain intensity were assessed using a pressure algometer and a numeric rating scale. The ROM was measured using a universal goniometer and MIMS was measured using an isokinetic dynamometer. Relative reliability was estimated using intra class correlation coefficient (ICC), and absolute reliability using standard error of measurement (SEM). Minimum detectable change (MDC) was calculated from SEM. Results The ICCs for PPTs ranged from 0.88-0.97, with SEM values ranging from 12.0 to 28.2 kPa and MDC ranging from 33.2 to 78.2 kPa. The ICCs for ROM ranged from 0.66-0.97, with SEM values ranging from 3.0 to 7.5˚and MDC ranging from 8.4 to 20.8˚. Finally, ICCs for MIMS ranged from 0.62-0.92, with SEM values ranging from 0.03 to 0.07 Nm/Kg FFM and MDC ranging from 0.09 to 0.19 Nm/kg FFM. Conclusion The results of this study indicate that PPTs, ROM and MIMS can be measured reliably on the affected shoulder in BCS with pain after treatment. This offer the possibility of using these measures to assess the effectiveness of interventions in this population.

Advancements in motion sensing technology can potentially allow clinicians to make more accurate range-of-motion (ROM) measurements and informed decisions regarding patient management. The aim of this study was to systematically review and appraise the literature on the reliability of the Kinect, inertial sensors, smartphone applications and digital inclinometers/goniometers to measure shoulder ROM. Eleven databases were screened (MEDLINE, EMBASE, EMCARE, CINAHL, SPORTSDiscus, Compendex, IEEE Xplore, Web of Science, Proquest Science and Technology, Scopus, and PubMed). The methodological quality of the studies was assessed using the consensus-based standards for the selection of health Measurement Instruments (COSMIN) checklist. Reliability assessment used intra-class correlation coefficients (ICCs) and the criteria from Swinkels et al. (2005). Thirty-two studies were included. A total of 24 studies scored “adequate” and 2 scored “very good” for the reliability standards. Only one study scored “very good” and just over half of the studies (18/32) scored “adequate” for the measurement error standards. Good intra-rater reliability (ICC > 0.85) and inter-rater reliability (ICC > 0.80) was demonstrated with the Kinect, smartphone applications and digital inclinometers. Overall, the Kinect and ambulatory sensor-based human motion tracking devices demonstrate moderate–good levels of intra- and inter-rater reliability to measure shoulder ROM. Future reliability studies should focus on improving study design with larger sample sizes and recommended time intervals between repeated measurements.

Objectives: Shoulder pain and impairment is a prevalent and disabling condition. While some Mulligan mobilization with movement (MWM) techniques have been shown to have beneficial effects, Hand Behind Back (HBB) MWM has not been investigated. The aim of this study was to investigate the effects of HBB MWM on shoulder pain, impairment and disability. Methods: We conducted a double blind randomized controlled trial in 44 subjects with shoulder pain and movement impairment presenting to an Indian general hospital. Subjects were allocated to receive either MWM and exercise/hot pack (n=22) or exercise/hot pack alone (n=22). The primary outcome was HBB range of motion (ROM). Secondary variables were shoulder internal rotation ROM, pain intensity score, and shoulder disability identified by the Shoulder Pain and Disability index (SPADI). All variable were evaluated by a blinded assessor before and immediately after 9 treatment sessions spread over 3-weeks. Results: A total of 60 patients were screened and 44 randomized. The average duration of symptoms was 4.1 and 4.7 weeks in the exercise and MWM groups respectively. Paired t-tests revealed that both groups demonstrated statistically significant improvements (p< 0.001) with large effect sizes for all variables. However, for all variables the MWM with exercise group showed significantly greater improvements (p< 0.05) than the exercise group. HBB ROM showed a mean difference of 9.31˚ (95% CI 7.38 to 11.27), favoring greater improvement in the MWM with exercise group. Conclusions: Shoulder HBB MWM with exercise improves outcomes in patients with acute shoulder pain and disability greater than exercise/hot packs alone.
