Patent ductus arteriosus: to treat or not to treat?

Abstract: Persistent patency of the ductus arteriosus in the preterm infant is associated with numerous morbidities, including higher rates of bronchopulmonary dysplasia and increased mortality. These strong associations have led to widespread use of cyclooxygenase inhibitors and surgical ligation to achieve ductal closure in the expectation that closing the ductus will reduce these complications. Each of these interventions has its own associated adverse effects. Neither individual randomised controlled trials nor meta… Show more

“…As in other studies the current data showed a significant association between PDA and BPD (p=0.002) (Table 4) [6,9,15,20,21,25,41,42,47,[66][67][68][69]. An almost similar risk was reported by Marshall et al in VLBW infants (BW <1500 g) [69].…”

“…An almost similar risk was reported by Marshall et al in VLBW infants (BW <1500 g) [69]. In preterm infants with RDS, PDA induces an interstitial and alveolar pulmonary edema, decreases pulmonary compliance, increases the need and length of mechanical ventilation and oxygen needs, thus increasing the risk for BPD [5,42].…”

“…Por lo tanto, la asociación entre el DAP y morbilidades graves que se registran con mayor frecuencia a menor EG como hemorragia intraventricular, hemorragia pulmonar, NEC, DBP y (22) muerte son difíciles de interpretar . A su vez dichas morbilidades pueden ser condicionadas por el (2) manejo clínico .…”

“…6 There are pharmacokinetic data regarding the use of indomethacin and ibuprofen to close ductus arteriosus in preterm infants. Several studies have shown the effectiveness of these agents and, beyond current controversies, 7 their use is still frequent especially when there is hemodynamic compromise. Both drugs cause adverse events, 8,9 such as renal function disorders, decrease in the mesenteric and cerebral blood flow velocities, and increase of platelet aggregation; likewise, ibuprofen interferes with bilirubin-albumin binding in plasma.…”
