Pathophysiology of Airway Colonization in Critically ill COPD Patient

Abstract: Although noninvasive ventilation (NIV) use in severe acute exacerbation of COPD has substantially reduced the need for intubation, an important number of COPD patients still are mechanically ventilated through a tracheal tube in the ICU. Intubation is a major risk factor for lower respiratory tract colonization (LRTC) in ICU patients. Other risk factors for LRTC include colonization of the oral cavity, nasopharynx, and gastric content. Aspiration of contaminated oropharyngeal secretions is increased by supine … Show more

“…Additionally, ETT can be responsible for mucosal lesions. Finally, ETT-induced loss of cough reflex suppresses the physiological clearance of microrganisms from the tracheobronchial tree [9]. Regarding the duration of exposure to conventional MV, it has been shown in early studies that the longer the duration of conventional MV, the greater the risk of VAP [10,11].…”

“…Lower airways colonization can also result from direct inoculation, during manipulations of respiratory equipment, such as endotracheal suctioning, or nebulization [17,23]. Aerosolized bronchodilators are routinely used in COPD patients with acute exacerbation requiring mechanical ventilation [34].…”
