This study examined the feasibility and outcomes of a training designed to enhance pediatric residents’ trauma-informed practices in primary care. Paired samples t tests examined changes in 33 residents’ attitudes, perceived competence, and perceived barriers toward trauma-informed care after a 2-hour training. Fisher’s exact tests measured changes in residents’ screening and referral behaviors. A subsample (n = 9) of residents were interviewed about the training. Residents reported increases in favorable attitudes ( P = .065) and perceived competence ( P < .001) and decreases in perceived barriers ( P = .001 to .521) to implementing trauma-informed care practices. Chart reviews revealed a significant increase in completed trauma screens (0% to 8.0%, P < .001) but no difference in referrals for psychology/psychiatry services (1.9% to 4.2%, P = .200). Residents reported finding the training helpful. Although residents were willing and understood the utility of assessing for trauma, they faced substantial barriers.

Fifty years of scholarship by behavioral science faculty shaped the training and practice of family physicians. One can trace behavioral science influence from the philosophical underpinnings of family medicine (whole-person, patient-centered, biopsychosocial care), to the translation of psychotherapy skills into tools for family physicians, to the discovery of successful, integrated healthcare teams of family physicians and behavioral science professionals. Family medicine as we know it today rests on the shoulders of scholars who collaborated to conceive, experiment, evaluate, improve, and innovate. The purpose of this paper is to recruit you – behavioral science professionals – to continue this tradition of scholarship in Family Medicine. To advance your scholarly efforts, start by understanding that “scholarship” is not limited to research. “Scholarship” is sharing your good work with other professionals; through that interchange, you and they can improve. Next, surround yourself with people who will help you learn and succeed: mentors, collaborators, and helpers. Select a focus for your scholarship and read scientific literature for a thorough understanding of the topic. Identify a methodology that fits your values, interests, skills and resources. Start small, then grow. Sharpen your skills in writing and, if necessary, in statistics. Finally, Act Scholarly! by sharing your work with other behavioral science scholars.

Hepatitis C virus (HCV) infection remains a pressing public health issue. Identification of long term infection in primary care settings and community health centers can facilitate patients’ access to appropriate care. Given the increase in HCV prevalence in the United States, improving the HCV care continuum and expanding medication access to disproportionately affected populations can help reduce disease burden, health care system costs, and transmission. Innovative treatment programs developed in the primary care setting are needed to deliver quality care to meet the demand of those engaging in treatment. This article describes an HCV treatment program developed within a primary care federally qualified health center (FQHC) using physician assistants (PAs) and nurse practitioners (NPs) to address the high number of HCV positive patients identified at the clinic. An interdisciplinary care team was established to optimize patient experience around HCV care and treatment, using on-site primary care behavioral health consultants, an HCV treatment coordinator, and a 340B contracted specialty pharmacy. From January 2015 to April 2017, the Public Health Management Corporation (PHMC) Care Clinic medical providers referred 189 patients for HCV treatment. Of those referred, 102 patients successfully obtained a sustained virologic response (SVR), representing a 53.7% success rate from referral to cure. This treatment program successfully integrated HCV treatment in a patient population heavily affected by substance use and mental illness. Support and adoption of similar programs in primary care community health centers testing for HCV can help meet the clinical/behavioral needs of these marginalized populations.
