Patient- and person-reports on healthcare: preferences, outcomes, experiences, and satisfaction – an essay

Abstract: With the shift towards patient-centered healthcare, patient- and person-reports of health-related factors, including outcomes, are seen as important determinants for evaluating and improving healthcare. However, a comprehensive, systematic categorization of patient- and person-reports is currently lacking in the literature. This study aims at developing a new classification system with well-defined constructs for patients’ and persons’ self-reports on health and healthcare. A literature research and evaluation… Show more

“…9 Patient satisfaction is a multi-dimensional complex structure that is the result of a comparison of an individual's health care experiences against one's own subjective standards. 10 Some have defined patient satisfaction as appropriateness between the patient's expectations of ideal care and their perception of actual care. 11 Researchers believe that high satisfaction leads to faster physical and psychological recovery of patient.…”

“…By involving patients and generating patient-based evidence throughout research and development, product design and evidence generation is more likely to concentrate on the health outcomes that are most relevant to them (3, 6). To enable optimization of patient-oriented product development, the research to define the needs of patients and relative importance of certain attributes must precede the design of the phase 3 clinical development program (31, 32). Time is needed for creation and validation of new PRO instruments that may be needed to address the patient-relevant endpoints, so that they can be used in pivotal phase 3 trials.…”

“…At an early stage in the product lifecycle, the research questions and the required output from preference studies may well differ from those in later stages of development. Therefore, the choice of preference research methodology may also differ from the methods typically utilized for preference studies conducted closer to the time of submission to authorities (31, 77). For example, in a new, previously unexplored disease one may be confronted with multiple attributes of interest that may exceed the reasonable number to test in a discrete choice experimental design (typically up to ~7 attributes).…”

“…Consequently, patient reported outcome measures (PROMs) are recommended to enable standardization of the data collected and provide accurate representation of the patients' subjective opinions of their functional capabilities. It is increasingly emphasized that the overall assessment of health status should include the subjective consideration of functional status, particularly the use of PROMs, together with objective testing [2].F These PROMs enable detailed planning of treatment and rehabilitation programs as well as the determination of the effectiveness of medical or rehabilitation interventions [3]. However, the measurements made are only as good as the tools that are used and the clinimetric considerations of both the psychometric and practical properties must both be fully investigated using the international guidelines such as Consensus-based Standards for the selection of health status Measurement Instruments (COSMIN) [4].…”
