Patient Harm from Repetitive Blood Draws and Blood Waste in the ICU: A Retrospective Cohort Study

Abstract: Introduction: Frequent blood testing in the intensive care unit (ICU) is instrumental to patient diagnosis, monitoring, and titration of invasive therapies. However, there is a growing appreciation that a significant proportion of ICU blood tests are reflexive and unnecessary.1,2 Serial phlebotomy is associated with extended hospital length of stay and acquired anemia in the general hospital population.3 Since patients in the ICU are prone to developing anemia,4,5 they are likely at increased ri… Show more

“…The range of HHb concentrations is chosen to investigate sensitivity of the MIP in diluted blood samples, which would enable decreasing the sample volume up to a factor 1000, thus strongly reducing medical waste in clinical applications. [76,[103][104][105][106][107] Figure 3b shows the calibration curve of the MIP-and NIP-coated PSiO 2 scaffolds. Two linear regions are evident for the MIP-based sensor, namely, a higher-sensitivity region between 0.1 and 1 mg mL −1 and a lower-sensitivity region between 1 and 8 mg mL −1 .…”

“…In case of significant blood volume loss, due to major surgery or prolonged hospitalisation [ 7 ], anaemia can develop or worsen, and it is usually associated with absolute or functional iron deficiency (the erythropoietic response is increased and outstrips the actual iron supply) [ 8 , 9 ].…”

“… 10 − 12 The frequency of measurements is limited ultimately by the frequency of blood draws which increase the likelihood of blood infections and anemia. 10 , 13 Meanwhile, there is substantial evidence suggesting that attentive monitoring and treatment of pH and lactate can improve patient prognosis and outcome. 14 , 15 In a multisite study with 348 patients having initial lactate ≥3.0 mEq/L, goal-oriented treatment targeting a 10% decrease in lactate per hour increased patient survivability by 10% as compared to the control (standard treatment, p = 0.067).…”

“…Current clinical standards for measuring blood pH and lactate require intermittent blood draws and analysis using benchtop instruments (Yellow Springs Instrument, YSI) or hand-held monitors (Abbott Laboratories i-STAT). The frequency of measurements is limited ultimately by the frequency of blood draws which increase the likelihood of blood infections and anemia. , Meanwhile, there is substantial evidence suggesting that attentive monitoring and treatment of pH and lactate can improve patient prognosis and outcome. , In a multisite study with 348 patients having initial lactate ≥3.0 mEq/L, goal-oriented treatment targeting a 10% decrease in lactate per hour increased patient survivability by 10% as compared to the control (standard treatment, p = 0.067) . In addition to reduced mortality, patients were discharged from the intensive care unit (ICU) earlier than those in the control group.…”
