Abstract: Monovision with modest refractive targets achieved good visual function and patient satisfaction without the inherent risk for troublesome visual symptoms associated with multifocal IOLs.

“…The performance obtained with vectographic tests of the IOL analyzed in our study is comparable to that obtained in previous studies on the same lens [12,[30][31][32], phakic patients with the same age-range [12] and monofocal IOL patients (mean of 57.1± 36.9 sec arc in 100 patients) [35]. Other ways to restore near vision with IOLs such as monovision (mean of 175.6± 1.13 sec arc in 26 patients) [36] and combining refractive and diffractive lenses in the same patient [33] show worse values of stereoacuity. Threshold stereoacuity analyzed properly with the Howard-Dolman method reveals different outcomes in comparison to ours and previous literature using vectographic tests.…”

“…Whether the dominant eye is corrected for distance or near may be less crucial. 53 In patients who are unhappy with their post-operative refractive surprise, regardless of cause, surgical management options include IOL exchange, a piggyback IOL (a sulcus IOL placed in front of the existing IOL in the bag) or refractive laser surgery.…”

“…22,23 Monovision can be implemented with contact lenses, 22,24,25 corneal refractive surgery, [26][27][28][29] or monofocal IOLs following crystalline lens removal in cataract surgery. 20,[30][31][32] Despite decades of clinical application, the ideal degree of anisometropia in monovision is still a topic of active research. 19,20,33,34 The optimal power difference between the two eyes has been reported to vary between 1.0 and 2.0 diopters (D), depending on the need according to the patient's lifestyle or occupation.…”
