Patient‐specific titanium onlay eminoplasty: A novel protocol for treatment of recurrent temporomandibular joint dislocation

Abstract: PurposeThe present study aimed to introduce an accurate, less morbid and less time‐consuming approach that secures the stability of the condylar movement's range for treating recurrent TMJ dislocations.MethodsFive patients were treated by surgical implantation of bilateral patient‐specific titanium onlay eminoplasties; each implant was virtually designed using specific software and milled from grade V titanium blocks. Intraoperatively, each implant was fixed with three mini‐screws along the lateral aspect of t… Show more

“…Surgical techniques should be considered where non-surgical and minimally invasive techniques are not successful in avoiding recurrent dislocation [14]. Surgical methods for treatment of recurrent dislocations include eminectomy to facilitate spontaneous reduction [48][49][50][51][52][53], restrictive techniques for prevention of recurrence of dislocation (blocking or redressment procedures) [54][55][56][57], and surgical correction of capsular ligament complex [58][59][60][61]. After any surgical treatment, patients should, for a few days, eat soft foods only and refrain from opening the mouth widely [17,24,41,50,57,60].…”

“…Entretanto, em caso de insucesso, a cirurgia aberta da articulação deve ser considerada, justificando a realização da eminectomia para o caso apresentado. Existem diversas abordagens cirúrgicas para a luxação recidivante da ATM e a escolha da técnica ideal ainda é uma questão discutível, tendo em vista que a maioria dos protocolos cirúrgicos demanda morbidade da região, tempo intraoperatório longo e risco de resultados imprevisíveis (IHAB et al, 2020).…”

Recently patient-specific implants were widely applied in craniofacial regions including eminence prosthesis [24] and mandibular reconstruction devices [22,28] . The use of patient specific implants for midfacial defects was reported in combination with a vascularized flap [29] , other than that it has not been largely found in literature.…”
