Abstract: This is a retrospective cohort study comparing patients who received chronic obstructive pulmonary disease (COPD) standard of care (SOC) in the preintervention cohort versus the postintervention cohort who received a case manager-led COPD care bundle at a postacute care (PAC) facility.
