Background Ethics in health care and research is based on the fundamental principle of informed consent. However, informed consent in geriatric dentistry is not well documented. Poor health, cognitive decline, and the passive nature of many geriatric patients complicate this issue. Methods The authors completed this systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The authors searched the MEDLINE PubMed, Web of Science, PsycINFO, and Cochrane Library databases. The authors included studies if they involved participants 65 years or older and discussed topics related to informed consent beyond obtaining consent for health care. The authors explored informed consent issues in dentistry and other biomedical care and research. Results The authors included 80 full-text articles on the basis of the inclusion criteria. Of these studies, 33 were conducted in the United States, 30 addressed consent issues in patients with cognitive impairment, 29 were conducted in patients with medical issues, and only 3 involved consent related to dental care or research. Conclusions Informed consent is a neglected topic in geriatric dental care and research. Substantial knowledge gaps exist between the understanding and implementation of consent procedures. Additional research in this area could help address contemporary consent issues typically encountered by dental practitioners and to increase active participation from the geriatric population in dental care and research. Practical Implications This review is the first attempt, to the authors’ knowledge, to identify informed consent issues comprehensively in geriatric dentistry. There is limited information in the informed consent literature covering key concepts applicable to geriatric dentistry. Addressing these gaps could assist dental health care professionals in managing complex ethical issues associated with geriatric dental patients.

During the pre-operation period, surgical candidates experience situations that stimulate psychological anxiety leading to stress during and after surgery which is known as preoperative anxiety. This condition can cause psychological and physiological adverse effects on both children and adults. Due to the high prevalence and adverse effects of preoperative anxiety, different treatments have been evaluated including pharmacological and non-pharmacological approaches. As pharmacological treatments may cause adverse effects such as breathing problems, drowsiness, interfering with anesthetic drugs, and prolonged recovery, non-pharmacological interventions are becoming more popular. These methods include cognitive-behavioral therapy, music therapy, pre-op preparation video, aromatherapy, hypnosis, guided imagery relaxation therapy, and massage. In this study, the most popular non-pharmacological approaches to preoperative anxiety are reviewed focusing on more recent evidence provided by clinical studies. The reviewed clinical evidence on the mentioned methods shows the efficacy of non-pharmacological interventions for the treatment of preoperative anxiety, so they can be used in patients of different ages and types of disease and surgery.

Anestezi uygulamaları ile her gün karşı karşıya olan ameliyathane çalışanlarının kendileri ya da yakınları ile ilgili bir cerrahi gerektiğinde hangi anestezi yöntemini tercih ettiklerini ve nedenlerini ortaya koymak. Gereç ve Yöntem: Yarı yapılandırılmış sorulardan oluşan bir anket, teknolojik iletişim yöntemleri ile (internet üzerinden Google forms, messenger, whatsapp gibi) Türkiye'nin farklı bölgelerinde ve farklı hastanelerinde görev yapan 1000 ameliyathane çalışanına ulaştırıldı. Yaş, cinsiyet, meslek ve süresi, kendilerine anestezi uygulanıp uygulanmadığı, uygulandı ise yöntemi, hangi anestezi yöntemini neden tercih edecekleri ya da etmeyecekleri ve hangi anestezi yöntemini neden önereceklerini kapsayan sorular soruldu. Bulgular: Yaşları 20 ila 60 arasında değişen katılımcıların %51.2'sini (n=327) erkekler, %48.8'ni (n=312) kadınlar oluşturuyordu. Katılımcıların % 69'u (n=440) herhangi bir nedenle kendilerine anestezi uygulandığını belirtirken %31'i (n=199) anestezi uygulanmadığını ifade etti. Katılımcılar için genel anestezi, rejyonal anestezi, periferik sinir bloğu yöntemlerinin üçünün de uygun olduğu cerrahi işlemlerde tercihin % 47 ile rejyonal anestezi (spinal, epidural, kombine spinal epidural anestezi) yönünde olduğu anlaşıldı. Yakınlarınıza ve hastalarınıza hangi anestezi yöntemini tavsiye edersiniz sorusuna ameliyathane çalışanlarının cevabı rejyonal anestezi ağırlıklı olmuştur. Sonuç: Bu çalışmada Türkiye'de ameliyathane çalışanlarının daha çok rejyonal anestezi tercih ettiğini saptadık. Daha geniş katılımlı çalışmalar ile anestezi tercihlerinin ne yönde olacağının öğrenilebileceği ve bu durumun iş gücü ve maliyet planlaması açısından da önemli olabileceği kanısındayız.
