Patterns and Effects of Admission Hyperglycemia and Inflammatory Response in Trauma Patients: A Prospective Clinical Study

Abstract: Background The constellation of the initial hyperglycemia, proinflammatory cytokines and severity of injury among trauma patients is understudied. We aimed to evaluate the patterns and effects of on-admission hyperglycemia and inflammatory response in a level 1 trauma center. We hypothesized that higher initial readings of blood glucose and cytokines are associated with severe injuries and worse in-hospital outcomes in trauma patients. Methods A prospectiv… Show more

“…Although the sample size fells within ranges of those found in the literature with similar objectives (39), the main limitation of the study was the poor generalizability of the sample to non-married individuals as less than 30% were single or divorced/widowed. This has an impact on the PTSD development as the Arab culture is known be a collectivist culture where social support is naturally provided, thus decreasing the incident of PTSD development (40).…”

“…Third, the primary outcome included sudden mortality, defined as 3-day in-hospital mortality. This endpoint was chosen in line with similar studies, an uncontroversial outcome related to prehospital care [6,12,26,28,29]. All nonsurvivor cases were double-checked by the PI, thus ensuring the accuracy of the primary outcome.…”

“…Likewise, fast bedside results are technically feasible due to ruggedized and small-sized devices designed and operated in a user-friendly way; point-of-care testing is still not a widespread standard procedure in EMS [24]. Accordingly, EMS should strive to introduce bedside analytics, and among all potential biomarkers, lactate has demonstrated, alone [13,14,25,26] or combined with other biomarkers and/or physiological variables [27,28], a significant predictive performance to determine high-risk patients in terms of tissue hypoperfusion. Second, the data extractors were not blinded.…”

“…The potential mechanisms underlying our findings are not totally clear but may include the following. First, admission hyperglycemia is correlated with more severe surgical injuries and inflammation ( 47 ). Surgery-related disease and stress response might have produced harmful effects on long-term outcomes.…”
