Patterns of Exacerbations of Chronic Non-Infectious Uveitis in Pregnancy and Puerperium

Abstract: Pregnancy is associated with lower numbers of flare-ups of non-infectious uveitis compared to the non-pregnant state. If flare-ups do occur during pregnancy, they happen predominantly in the first trimester.

“…14 From the clinical report in 1969, we observe that the patient's illness began as an anterior iridocyclitis in one eye, probably precipitated by the postpartum reduction in steroid hormones that favor inflammatory processes. 15,16 The inflammation then cleared, only to reappear with the second labor. This time, however, the disease involved both eyes and was in the form of a massive, non-granulomatous chorioretinitis as well as iridocyclitis.…”

“…This time, however, the disease involved both eyes and was in the form of a massive, non-granulomatous chorioretinitis as well as iridocyclitis. Considering pregnancy and childbirth as an immunocompromised state, 16 and taking into account that it has been described that bilateral herpetic ocular compromise is more common in immunocompromised patients, 1 we want to highlight that our patients had received systemic immunomodulatory medications that could have favored the extension and expression of ocular herpetic infection bilaterally. Some of the drugs mentioned in Table 1 are not known to specifically modify viral immune response but FIGURE 5.…”

“…They confirmed a beneficial effect of pregnancy by reducing the rate of relapses from 2.4 to 1.0 (p < 0.001) and noted that relapses were more frequently observed in the first trimester [14]. These results were recently confirmed by Chiam et al, who observed a marked decline in the number of uveitis flare-ups during the second and particularly the third trimester of pregnancy; whereas the rates of flare-ups were similar to the non-obstetrical period in the first trimester and the postpartum [15].…”

“…Inflammatory disorders such as rheumatoid arthritis, sarcoidosis, and spondyloarthropathy have both systemic and ocular manifestations which notably decrease during pregnancy [57]. The decreased symptomatic presentation of these pathologies may be due to rise of corticosteroids found in pregnancy.…”
